Newent Doctors Practice has an exciting opportunity for a Practice Nurse to join our expanding doctors practice in Newent, which is just 8 miles from the city of Gloucester on the Gloucestershire / Herefordshire border. We are looking for an enthusiastic, committed and highly motivated individual to join our forward thinking clinical team and provide excellent medical care for our patients.

We are offering a position of 24 hours a week across three days, Monday, Tuesday and Thursday 9:00am-6:00pm.

Please note you will be expected to support our enhanced service access clinics and work a late shift (11:00am-8:00pm) every fourth Thursday evening and approximately 3 Saturday mornings a year.

Salary is in the range of £16.61 -£19.16 per hour depending on qualifications and experience.

Main duties of the job

Our preferred candidate would be an experienced practice nurse, with knowledge of chronic disease management, in particular diabetes and respiratory illness and all aspects of practice nursing including child health, cervical cytology, health promotion, wound care, and infection control etc.

They will work as a senior member of the practice multidisciplinary team, and will ensure nursing services are delivered effectively to the entitled patient population.

Newent Doctors Practice is an ten general practitioner training practice with approximately 11,500 patients. in addition to this, we have a team of 42 staff, including Receptionists, Pharmacist, Pharmacy Technicians, Dispensers, Nurses, Advanced Nurse Practitioner, First Contact Physiotherpaists, First Contact Paramedic, Mental Health Practitioner Healthcare Assistants and Phlebotomists, Secretarial and Administrative Staff, Reception Team Leader, Data Quality Improvement Officer, Operations Manager and Practice Manager.

Job description

Job responsibilities

The following are the core responsibilities of the practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. To lead or support the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing and or supporting the implementation of audits, training, support and evidence of compliance to the practice manager

b. To lead or support the production of the IPC annual statement in conjunction with the practice manager

c. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

d. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

e. To develop, implement and embed health promotion and wellbeing programmes

f. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

g. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

h. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

i. As required, to diagnose and manage acute and chronic conditions

j. To prescribe medication for patients in accordance with PGDs and own scope of practice

k. To review patient medications to enhance compliance

l. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

m. To co-ordinate the provision of travel medicine services

n. To request pathology services and process pathology results as required

o. Where necessary, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs.

p. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

q. To maintain accurate clinical records in conjunction with extant legislation

r. To ensure read codes are used effectively by all members of the nursing team

s. To ensure the appropriate maintenance and use of chronic disease registers

t. To develop, implement, embed and participate in well woman clinics

v. To ensure compliance with the practice chaperone policy

ee. To support the clinical team with all safeguarding matters, in accordance with local and national policies

ff. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

gg. To deliver opportunistic health promotion where appropriate

hh. In participation with the practice manager and nursing colleagues, ensure the maintenance of equipment and stock levels relating to patient care.
