Job summary
Due to staff retirement and our growing practice we are looking for a Practice Nurse to work approximately 30 hours per week. We are an
innovative, supportive and forward thinking training practice with various
outside interests. We moved to a new purpose built health care facility in November 2020. We have 15,000 patients and are
focussed on high quality patient care and employee development.
We have an
excellent skill mix team including our friendly and experienced nursing/treatment room team.
Main duties of the job
We are looking to welcome an experienced, qualified Practice Nurse to our team to assess, plan, develop, implement and evaluate treatment programmes for our patients. The ideal candidate will have experience in the management of long-term conditions and be able to undertake a range of practice nurse clinical duties. You will be aware of and act in accordance with national policy procedures, legislation and NMC standards and be on the nursing register.

We have a large and diverse clinical team of GP's, Clinical Practitioners, Paramedic, Practice Nurses and HCA's as well as a large Patient Services and Administration Team.
We are a member of the Brackley and Towcester Primary Care Network and have the services of Clinical Pharmacists, First Contact Physiotherapists, Social Prescribers and a Mental Health Worker.

plan and deliver skilled nursing care within the practice as a member of the Treatment
Room team. To undertake a range of practice nurse clinical duties. To be aware
of and act in accordance with national policy procedures, legislation and NMC
standards and to ensure is on the nursing register.
- Main duties:
- To assess
patients health needs and implement nursing interventions to meet their
needs.
- To manage the
care and treatment of patients with stable or long term conditions within
agreed protocols.
- To maintain high
standards of infection control at all times, protecting patients, self and
colleagues.
- To actively
engage patients and carers in the management of their health, promoting
health and wellbeing.
- To refer
patients to other members of the practice clinical team in line with
agreed procedures.
- To maintain
accurate records of all clinical activity in line with practice protocols
and NMC standards.
- To assist in
specialist clinics within practice protocols.
- To contribute to
clinical governance activities within the team including audit and
evaluation.
- To support other
members of the practice team in maintaining health, safety and security,
identifying and assessing potential risks and contributing to management.
- To limit your
actions to those which you feel competent to undertake. If you have any

CLINICAL DUTIES:
Long Term Conditions
- Work with other health care professionals to monitor, manage and treat long-term conditions in line with national and local policies and practice needs.
Health Promotion
- To
recognise patients readiness to change in supporting their attempts to change
unhealthy behaviour.
- To
provide healthy eating advice individually in opportunistic consultations.
- To
provide smoking cessation advice individually in opportunistic consultations
and refer to the Smoking Cessation service.
- To
provide individual advice to patients about increasing activity or exercise.
- To
offer advice on recommended safe limits for alcohol consumption.
- To
assess, plan, provide and evaluate nursing care to meet the care needs of
individuals and groups in the practice population.
Emergency Management
1.
To
be competent in anaphylaxis and resuscitation techniques.
Child
Health/Protection
- To
recognise the signs, symptoms and categories of child abuse.
- To
understand the importance of recognition, observation, documentation and
communication channels.
- To
be aware of statutory child health procedures and statutory local guidance and
basic legal issues.
- To
be aware of other practitioners and agencies roles.
Family Planning
- To
advise patients on available methods of contraception and refer patients as
appropriate.
- To
perform interval health checks for patients using hormonal contraception.

advise patients on reducing the risk of sexually transmitted diseases.
Womens Health
- To
advise patients on the national and local cervical cytology and mammography
screening programmes.
- To
advise on breast self-awareness.
- To
undertake cervical cytology sampling.
- To
take swabs for microbiology.
- To
give patients advice on the increased peri and post-menopausal risk of
osteoporosis
Mens Health
1.
To
raise awareness and give basic advice on issues covering:
- Testicular
self-examination
- Continence and
prostatic disease
- Family planning
- Sexual Health
Vaccinations and
Immunisations
- To
administer injections under an individualised prescription or Patient Group
Directive (PGD).
- To
ensure safe storage, rotation and disposal of vaccines.
- To
provide an adult and childhood immunisation service.
- To
maintain a call and recall system for patients.
- To
work to PGD for prescription only medicines.
Infection control
1.
To
apply infection control measures within the practice including:
- Universal hygiene
precautions
- Collection and
handling of laboratory specimens
- Segregation and
disposal of waste materials
- Decontamination of
instruments and clinical equipment

treatment of sharps injuries
- Dealing with blood
and bodily fluid spillages
