Job summary

This is an opportunity to join an established GP Practice in a Nuneaton suburb. We are looking to expand our current workforce with a dynamic, energetic, enthusiastic and forward thinking Advanced Nurse Practitioner or Physician Associate who puts patient care at the heart of what they do on a daily basis.

Main duties of the job

Working autonomously and is accountable for own professional actions to assess, diagnose, plan, implement and evaluate treatment/interventions and care. Patient consultations will be undertaken face to face or via telephone and video calls.

Job description

Job responsibilities

  • Assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis.
  • Undertake telephone/video triage/consultations of patients requesting urgent appointments
  • Support the GPs with the development and delivery of online consultations
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnose, plan, implement and evaluate interventions / treatment for patients with complex needs. Proactively identify, diagnose and management treatment plans for patients at risk of developing a long term condition
  • Where appropriate undertake patient referrals for further care to other health professions either within and outside of the Practice and where required directly admit patient to hospital in acute medical need
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan
  • Work with patients in order to support adherence and compliance with prescribed treatment. Where appropriate, provide advice and information on prescribed or over the counter medication, medication regimes, side effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient win complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Keeping abreast of clinical changes appropriate to the level at which they are working
  • To actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and to escalate concerns about radicalisation if identified
  • Support the practice in the delivery of its seasonal flu vaccination programme or covid-19 vaccination programme or similar pandemic programme as and when required
