Main duties of the job

The focus of the role is ensuring the delivery of safe and effective nursing care to our patient population. The post holder will be the nurse team leader at Testvale Surgery and will also be able to work as part of the whole general practice as well as autonomously, demonstrating strong professional decision making skills and clinical expertise. They will be responsible for ensuring the delivery of evidence-based nursing care to the Practice population and will actively support the Practice management team in developing and reviewing clinical policy.

The Lead Practice Nurse will be responsible for several clinical areas such as Infection Control, Health Promotion and Chronic Disease management.

Testvale Surgery has its roots in Totton going back over a century and since 1987 has been at our present location on Salisbury Road. We are passionate about the care we provide for our patients and always try to offer convenient appointments and the highest quality care. We have well-established charitable support in the Friends of Testvale Surgery, who help us to improve the care we provide for our patients.

The premises have gradually expanded to provide a fully equipped and staff service for primary medical care. We comprise seven partner GPs and two salaried GPs and a compliment of seven members of the Nursing, Health Care Assistants and Phlebotomists, as well as a admin support team.

We are an established training practice and regularly have medical students and GP registrars.

JOB SUMMARY

The post holder will be the nurse team leader at Testvale Surgery and will also be able to work autonomously, demonstrating strong professional decision-making skills and clinical expertise. They are responsible for ensuring the delivery of evidence-based nursing care to the Practice population and will actively support the Practice Management team in developing and reviewing clinical policy. They will provide leadership to the nursing team to ensure the effective and efficient provision of Primary Health Care services.

ROLE PURPOSE

To work as an autonomous practitioner providing care to patients within the primary care setting using specialist practice nurse skills To provide clinical leadership within the nursing team To develop the clinical staff within their roles ensuring appropriate mentoring and support. To organise and co-ordinate the provision of nursing services for the Practice To work collaboratively with the whole practice team ensuring that the care delivered to the Practice population is relevant, responsive, of the highest standard and is clinically effective and efficient To work within the framework of the NMC Code of Professional Conduct and Scope of Professional Practice to ensure that the best possible care is given to the Practice population.

ROLE RESPONSIBILITES

  • Maintain patient confidentiality and work within practice guidelines and protocols at all times
  • Demonstrate clinical excellence in practice nursing, adhering to national and local policies/guidelines/protocols and Patient Group Directives (PGDs)
  • Initiate and arrange a methodical assessment of patient and service needs
  • Work within an integrated and multidisciplinary team environment using appropriate referral pathways
  • Using clinical software available, ensure targets and quality indicators in the new GMS contract are met
  • To be aware of health and safety issues affecting staff, patients and visitors and keep up to date with health and safety training
  • Decide on appropriate wound dressing for individual patients and recognise when a patient requires a change of dressing or further intervention e.g. referral to GP/ /podiatry etc
  • Maintain and record accurate patient records on the computer
  • Give health prevention advice to patients with wound infections, non-healing wounds, healthy eating and stop smoking

To make and accept referrals within the team as appropriate

Clinical Duties

Provide clinical excellence in all treatment room areas of routine general practice nursing as part of GMS and national guidelines

Opportunistically promote a healthy lifestyle and counsel patients as necessary, enabling them to make effective lifestyle changes

Assess, plan, implement and evaluate care pathways to meet the needs of individuals and groups within an agreed policy, PGDs, guidelines, national service frameworks and identifying the need for referral to others

Assist and chaperone GPs, when necessary, with the examination and treatment of patients

To ensure resuscitation/anaphylactic shock equipment is safely maintained in your room

To maintain adequate stock control of dressings etc.

Make professional autonomous decisions for which the post holder is accountable for and to provide and promote safe, effective and cost-efficient individualised patient care

Where necessary evaluate patients test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated.

Communicate effectively with patients/families/carers regarding information about their care and considering the equality and diversity needs and preferences of said population.

Maintain accurate and contemporaneous records using the practice clinic system (EMIS)

Protect and promote the health and well-being of patients, their relatives and carers through advice, education, screening and signposting to appropriate agencies when required

Undertake clinics and offer focused lifestyle advice in line with local, national guidance and quality outcome framework indicators

Manage programmes of care for patients with long term conditions and other practice nurse duties by providing and evaluation care under agreed guidelines and patient group directives. These may include but not exhaustive of:

o Diabetes

o Cardiovascular disease

o Asthma, COPD

o Contraception and Sexual Health

o Adult and childhood immunisations

o Learning disabilities

Professional and Managerial Role

Development and maintenance of relevant policies and protocols that are within the Lead Practice Nurse role responsibility, appropriate delegation of above to other personnel, also ensuring they are maintaining such policy and protocol

Line manages staff members according to the Practice organisational plan.

Work with the Head of Clinical Services to maximise all areas of income for the Practice.

Minimise expenditure for the Practice whilst ensuring high standards of care are provided Develop, deliver and maintain primary nurse led services including long term conditions, maintaining disease registers in liaison with administrative staff

Lead the Infection, prevention control within the Practice, audits are performed in a timely manner and ensuring that changes and updates are reflected in the policies.

To ensure effective communication links between IPC clinical leads, links and Facilities Manager.

Ensuring any changes to policies are cascaded accordingly to the relevant practice team.

Performance Appraisal

Provide ongoing performance management, feedback and development to their team. Conduct annual and interim appraisal meetings and completion of appraisal documentation.
