Job summary
Teams Medical Practice has an exciting opportunity for an enthusiastic, forward-thinking GP to join our friendly and committed team as a GP Partner for 4-6 sessions per week in Spring 2023.
We are an established, high-achieving practice serving a socio-economically deprived population of approximately 5,500 patients in the Inner West PCN area of Gateshead.
We are keen to hear from GPs at any stage in their career and are happy to support career development for all staff.
We have a strong focus on staff wellbeing and hold regular clinical meetings. The Clinical team also meet for a daily catch up and strive to ensure that, as a group, we maintain a good work life balance.

Main duties of the job
The main duties will include undertaking surgeries with a
mix of telephone triage/consultations, eConsultations, face-to-face
appointments, and home visits when necessary.
You will undertake a share of duty /on-call sessions. In addition you will:
Action Repeat prescription queries & sign scripts
Action
clinical correspondence, pathology results and general queries
Referrals as per practice procedure
Liaise with relevant health & care services including hospital trust, community
trust & 3rd sector
Keep records to a recognised standard
Take responsibility
for continuing & on-going care of patients
Contribute
to supervision of trainees and medical students
Provide
clinical support to the wider team during duty sessions
Work
as part of the practice team to ensure continuous quality improvement, audit,
the improvement of systems & care management
Participate
in practice meetings
Produce medical
reports as required
Maintain a record of personal CPD as required for ongoing GP appraisal
About us
We are a GMS practice with a list size
of ~5,500 patients. The team
comprises 6 GPs, a Practice Manager, an Assistant Practice Manager, an established nursing team, and a reception team
of 10.
Our aim
is to provide high quality care at the point of need to our patients, many of whom live in areas of high deprivation. We endeavour to anticipate needs and intervene with preventative
strategies where possible.

Receives
patients with undifferentiated and undiagnosed problems and makes assessment of
their health care needs.
Consults
with patients in the surgery, at home visits and via telephone or on-line

Undertakes
telephone calls, home visits, checking and signing of repeat prescriptions and

protocols, develops care plans for health.
