Job summary

Forum Family Practice are looking to recruit a friendly, motivated, enthusiastic and experienced care navigator & prescription clerk for a minimum of 24 hours to join our team, due to increasing patient numbers.

To be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team. Duties can include, but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, the processing of information and assisting patients as required.

To act as the central point of contact for patients, be responsible for the distribution of information, messages and enquiries for the clinical team, liaise with multi-disciplinary team members and external agencies such as secondary care and community service providers.

To be responsible for the accurate and timely processing of prescription requests for the entitled population, liaising effectively with all relevant personnel.

Main duties of the job

  • Ensure all patients and visitors are greeted professionally and politely with their queries dealt with effectively and efficiently.
  • Answer all incoming calls from patients and other health care professionals. Divert or take messages as required.
  • Deal with administrative queries and requests from patients & GP's.
  • Maintain and monitor the practice appointments system. Make/re-arrange/cancel appointments as required.
  • Initiate contact with and respond to requests from patients, other team members and associated healthcare providers
  • Scan all patient information i.e. medical document sensoring all patient records are kept up to date.
  • Deal with referrals to primary and secondary care
  • Process and distribute incoming (and outgoing) post as well as dealing with other adhoc duties and general administrative tasks.
  • Answer our prescription line taking requests for medications and dealing with patient queries
  • Liaising with pharmacies and managing repeat dispensing tasks.

You will join a friendly clinical team of 5 Partner GPs, 2 ANPs, 2 Practice Nurses, 4 Health Care Assistants. In addition to support our practice through our Primary Care Network, we have assigned 2 Link workers, 3 Pharmacists, 1 Mental Health Practitioner & visiting Paramedic. We also have a medication technician team to support prescription queries and medication reviews.

The non-clinical team who will look after and support you, includes a managing partner, an office manager and a 12-person admin support structure.

Duties and Responsibilities of the Post:

Reception

  • To ensure that all visitors and telephone callers to the practice are greeted professionally and telephone calls are managed appropriately.
  • Making appointments on SystmOne, prioritising patient expectations and booking accordingly.
  • The post holder will action front desk tasks in a timely manner.

Prescription Desk & Tasks

  • The post holder will process prescription requests in a timely manner, liaising with the relevant other teams where needed. Other duties include actioning prescription tasks, promote online ordering for repeat medications.
  • The post holder must work as part of a team, have the ability to organise and prioritise their own workload on a day to day basis, and observe strict rules about confidentiality at all times.

Reports

  • To follow the protocol for logging and running patient reports as requested.
  • Ensure appropriate costs are aligned with each report.

Data Entry (in training)

The post holder will maintain a manageable workload that comes through the workflow, the workflow will be checked regularly and processed within the timescales set. Any items that need actioning followed up or investigating will be carried out within the correct processes, ensuring clinical data is directed to relevant teams with a clear guidance of expected outcome.

ADHOC Duties:

Any other duties requested by the partners or management.

Communication:

The post-holder should recognise the importance of effective communication within the team and will strive to:

  • Communicate effectively with other team members
  • Communicate effectively with patients and carers
  • Recognise peoples needs for alternative methods of communication and respond accordingly
