Job summary
To work as an autonomous practitioner carrying out assessments via telephone, video call and face to face including in patients home for patients presenting with undifferentiated and undiagnosed acute illness or with long term conditions. Providing care for the presenting patients from initial history taking, clinical assessment, ordering diagnostic testing, making necessary referrals, diagnosis, treatment, and evaluation of care. You will be expected to answer queries relating to ongoing patient care and medication reviews. Providing safe clinical practice working within boundaries of competence and skills, adhering to, and working within the NMC code of Conduct. To work collaboratively with the general practice team to meet the needs of patients supporting the delivery of policy and procedures.
Main duties of the job
Assess, diagnose, plan, implement and evaluate treatment/interventions for patients presenting with an undifferentiated diagnosis.
Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness.
Work with patients in order to support compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed, or over-the-counter medication on medication regimes, side effects and interaction.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
Undertake sexual health service procedures as appropriate to competence.
Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret finding /reports.
Refer patients directly to other services/agencies as appropriate.
Other areas of relevant clinical importance as required.
Assist the practice in providing all service contract undertaken e.g. GMS/PMS Enhance or essential services plus DES/ LES local authority (list not exhaustive).
About us
The Discovery Practice serves a population of 8800 providing GMS services. Our clinical team comprises 3 GPs and 2 ANPs and 1 Trainee ANP supported by our Nursing team of 2 RGN with Respiratory and Diabetes specialties and our 2 Health Care Assistants. We are part of Central Middlesbrough PCN which incorporates additional roles such as Physiotherapists, clinical Pharmacists and technicians, Social Prescriber and link with TEWV Mental health practitioners. As well as Care Co-Ordinator's all which compliment and add to the services we provide to our registered patients.
We are an innovative and developing practice which supports CPD training and mentorship and supervision. Friendly and supportive we wish to welcome a new member to our team.
Although advertising for a Advanced Nurse Practitioner we would also welcome any Nurse Practitioners who are wanting to pursue a prescribing qualification and would be willing to support any suitable candidates at various stages of qualification.
