Job summary

The candidate would be part of our growing MDT for Central Middlesbrough PCN. They will be supported in their role by our 7 member practice teams and our PCN additional roles team and Clinical director.

The role involves working within a team of health professionals who are enthusiastic to improve the health and wellbeing of our population through proactive care and improving health outcomes for our patients. The candidate will provide clinical care and empowerment of care home residents through weekly care home rounds and development of individual Personalised Care Plans.

They must be keen to support the PCN to develop innovative and successful ways of integrating care across wider health and social care organisations. Promoting patient centred care.

Main duties of the job

To support people entering or already resident in the PCNs aligned care homes who are a registered patient within one of the PCN practices.

To work with community providers and other relevant partners to establish and coordinate a Multi-disciplinary team to enable the development of personalised care and support plans to enhance the health of those registered patients in our care homes.

Working to develop models for proactive and preventative care to our patients.

Supporting with patient discharge from hospitals and transfers of care between settings.

Job responsibilities

Main Purpose of the Job

To work as an autonomous practitioner for Central Middlesbrough PCN in support of Enhanced Health in Care Homes. Carrying out assessments via telephone, video call, face to face and care home visiting of patients presenting with undifferentiated and undiagnosed acute illness or with long term conditions, specifically for Elderly population residing in care homes. Providing care for the presenting patients from initial history taking, clinical assessment, ordering diagnostic testing, making necessary referrals, diagnosis, treatment, and evaluation of care. You will be expected to answer queries relating to ongoing patient care and medication reviews. Providing safe clinical practice working within boundaries of competence and skills, adhering to, and working within the NMC code of Conduct. To work collaboratively with the PCN general practice team(s) to meet the needs of patients supporting the delivery of policy and procedures.

Main Responsibilities

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.

Clinically examine and assess patient needs and plan clinical care accordingly.

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed, or over-the-counter medication on medication regimes, side effects and interaction.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

Undertake sexual health service procedures as appropriate to competence.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret finding /reports.

Refer patients directly to other services/agencies as appropriate.

Other areas of relevant clinical importance as required.

Assist the practice in providing all service contract undertaken e.g. GMS/PMS Enhance or essential services plus DES/ LES local authority (list not exhaustive).

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working across the Central Middlesbrough PCN practices.

Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence.

Work effectively with others to clearly define values, direction and policies impacting upon care delivery.

Discuss, highlight and work with the team to create opportunities to improve patient care.

Manage and lead on the delivery of specifically identified services or projects as agreed with the clinical management team.

Provide and deliver training to develop the PCN/Practice teams.

Mentor and support other nurses in developing and maintaining clinical skills.

Contribute to the planning and implementation of the teaching for new and existing staff, such as medical students, NP students, GP registrars, health Care assistants, etc. within the practice.

Communication and Relationships

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Consider patients communication requirements.

Anticipate barriers to communication and take action to improve communication.

Maintain effective communication within the practice environment and with external stakeholders.

Act as an advocate for patients and colleagues.

Ensure awareness of sources and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Attend practice and external meetings as requested.

Maintain effective communication with the practice team.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
