Job summary

Due to a growing list size, we are looking for a full-time Practice Nurse/Nurse Prescriber to join our enthusiastic, friendly and well-established nursing team. We are a progressive thinking training practice with a great team ethos and a committed and supportive Primary Health Care Team.

The post holder will be responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of nursing and preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients and support the delivery of policy and procedures.

Main duties of the job

We are looking for a highly motivated, enthusiastic, flexible, self-directed practitioner with a minimum 2 years post-registration experience and at least 1 year recent primary and community nursing experience. The ideal candidate will have experience in management of long-term conditions and involvement in implementing and using protocols and clinical guidelines. Experience of minor injury is desirable.

About us

We are a 7 doctor training practice affiliated to both the Banbury and Northampton schemes. We have a well-established and experienced Nursing Team comprising of 3 Nurse Practitioners, 2 Health Care Assistants and a Phlebotomist as well as a large Administrative Team.

We are part of the Brackley and Towcester Primary Care Network. Through the PCN, we have introduced Clinical Pharmacists, First Contact Physios, Mental Health Worker and Social Prescribers to the Practice.

ACCOUNTABLE TO: Dr Rathborne

LOCATION: Springfield Surgery, Springfield Way, Brackley, Northants, NN13 6JJ

HOURS: 37 per week

The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of nursing and preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients and support the delivery of policy and procedures.

Essential key duties.

1. Assist in the organisation and co-ordination of the provision of all nursing services for the practice and developing nursing services to provide evidence based optimum practice.

2. Work together with the Duty Team to manage and provide care for patients with acute/urgent on the day needs.

3. Proactively identify, diagnose and formulate treatment plans for patients at risk of developing a long-term condition.

4. Work with other health care professionals to monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.

5. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols. Work with patients in order to support adherence to prescribed treatments

6. Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care

7. Implement and participate in vaccination and immunisation programmes for both adults and children. Advise, support and administer vaccinations where appropriate for patients travelling abroad.

8. Support and manage health needs of women presenting for family planning and cervical cytology consultations and other womens health issues.

9. Undertake telephone triage to assess patients and plan any treatment required, bringing patients in or referring to Doctor where appropriate.

10. Provide a mentoring role to colleagues when required.

11. Meet the needs of patients presenting for opportunistic wound care

12. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

13. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

14. Participate in the maintenance of quality governance systems and processes across the practice. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, presenting the results and implementing improvements where required

15. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Participate in the education/training of others including General Practice Registrars, medical and nursing students and Health Care Assistants.

16. Provide enthusiastic day-to-day support and mentoring for Health Care Assistants.

17. Participate in or lead Practice Meetings and Primary Health Care Clinical Team meetings.

18. Apply infection-control measures within the practice according to local and national guidelines

19. Provide cover for colleagues as and when necessary.

20. Triage and treat patients, making any necessary referrals to other members of the primary health care team.

21. To independently prescribe to patients whilst following agreed protocols.

22. When required work with the local community to provide health promotion information.
