Job summary
Wyre Forest Network of Independent Practices (WFNIP) PCN are a
Primary Care Network serving a population of 41,000 over 4 practices.
Even though the Covid pandemic has turned our normal practice
upside down, we are continuing to plan for the future and are committed to
providing a friendly, open and supportive environment where our staff feel
valued, safe, appreciated and comfortable with sharing ideas which could
improve and diversify our services further.
We are looking for motivated, friendly, clinically experienced Nurse practitioner for our Care Home. You will be expected to undertake home visits
to patients homes or Care Homes for patient consultations and risk assessments.
You would be expected to collaborate within the PCN and with other agencies in
order to make decisions which provide the best course of action for our
patients care.

We offer 5.4 weeks annual leave plus bank holidays, dependent on
years of service. We encourage and support further personal development. Salary
dependent on experience, NHS Pension entitlement. Must hold full UK driving licence and have access to own vehicle.

Main duties of the job
The Care Home
Visiting and Treatment Team is a multi-disciplinary team with specialist skills
in caring for older people in the community, whether in their own homes or a
residential setting.
The post
holder will be part of the team providing assessment and implementation of appropriate
care and treatment of these patients in conjunction with the Care Home and
Community Nurse Practitioner, Frailty Paramedic and the allocated
Nurse Practitioners from the Health and Care Trust Neighbourhood Team.

Job responsibilities
The post holder will be responsible for the assessment, planning and
provision of specialist nursing advice to residents of care homes. Being able to formulate and complete individual care plans.
They will ideally have experience of community working and working with vulnerable patients with complex care needs, such as dementia and have a good understanding of frailty.
Be able to work independently and manage a group of patients within the specialty whilst working as part of the multidisciplinary team, delivering individualised personalised care.
Be able to organise own caseload and practice, identifying and maintaining supportive networks for self and other staff members.
Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care, to include comprehensive history taking, physical examination, enabling formulating a plan of care.
- To initiate and carry out the agreed treatment plan.
- To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team.
Act as an advocate for patients, their families and care home staff to support and enable patients and carers to make informed decisions relating to their treatment and management, using advanced communication to skills to have difficult conversations, such as completing ReSPECT documentation.
Ideally possess a good understanding of QoF and how this affects the wider PCN.
Attend and participate in multi-professional team meetings.
- To have knowledge of a variety of therapeutic interventions including short term problem solving, crisis intervention and basic CBT techniques.
- To develop and maintain effective liaison and communication with external agencies such as GP, acute hospitals and the local authority.
- To participate actively in Service Development to enhance quality of care.
- To maintain accurate clinical documentation within the appropriate EMIS record.
- Be willing to undertake training to enhance their own skills, ie Dementia, Health Assessment
