Job responsibilities

We are a friendly and supportive well-established practice located in Low Grange Health Village, Middlesbrough. The practice operates from a purpose-built premise. Training practice for medical students from Newcastle and Sunderland and approved GP registrar trainer from August 2021. Highly recommended as teaching practice by Medical and Nursing students. Patient population is 26,500. Computer system is S1. CQC rating 'Good'. High QOF achievement.

Our team comprises of; - 11 GP Partners, 7 Nurse Practitioners, 9 Practice Nurses, 5 Health care Assistant, 3 Pharmacists, 1 Clinical Pharmacist (PCN) and a highly skilled and supportive administration and reception team with good staff retention.

We are the lead employer for Eston PCN with a Partner as the PCN Clinical Director. We are supported by; - social prescribers, pharmacists, Pharmacy technicians, Physician Assistants, a Care co-ordinator and a Mental Health Practitioner, cancer care co-ordinator, Health and well-being coach, Dietician, Alcohol Co-ordinator, CAMHS practitioner. We have strong working relationships with our practice attached community services, ICB and local practices. We are also part of ELM Federation. We provide extended hours access on an evening until 8pm and Saturday 9am - 5pm.

Our team hold regular clinical, practice and partnership meetings to support clinical learning, share information and develop services and new ways of working.
