COMMUNITY CARE HALL GREEN PCN
The post holder is a qualified pharmacist working within
their professional scope to support and collaborate with a team of pharmacists
in a General Practice setting. They will be guided, managed, and mentored by
the Medicines Management Lead.
In this patient-facing role, the pharmacist will be an
integral member of a multi-disciplinary team. Responsibilities will include
managing minor ailments, contributing to the care of patients with chronic
conditions, and conducting clinical medication reviews to support patients with
complex polypharmacy needs.
Additionally, the pharmacist will serve as a key resource
for Hall Green Health staff, addressing prescription and medication-related
queries. They will support the repeat prescription process, handle acute
prescription requests, perform medicines reconciliation during care
transitions, and enhance systems for safer prescribing. The post holder will
provide expert clinical medicines advice, supporting both the medical and
social care needs of patients across the GP practice(s).
