Job summary

The Corner Surgery has an exciting opportunity for a Care Coordinator / Receptionist to join our friendly, welcoming, and supportive team. We are looking for a hardworking and committed individual with a genuine interest in helping patients and contributing to the delivery of high quality healthcare.

The successful candidate will enjoy working as part of a team, have a positive and caring approach, and take pride in providing an excellent service to our patients.

This role is for all day Wednesday and Friday, with some flexibility required to provide cover during annual leave and sickness, as needed.

We are hoping to interview candidates before the closing date and therefore reserve the right to close the advert early should a suitable candidate be found.

Main duties of the job

Care Coordinator duties

  • Answer the telephone promptly and deal with queries appropriately.
  • Book appointments and home visits, ensuring that patients are offered the most appropriate appointment for their need.
  • Signpost patients to more appropriate services when necessary
  • Cancel and rearranging appointments when necessary.
  • Process requests for prescriptions and liaising with the pharmacy team.
  • Deal with patient requests regarding results of diagnostic tests
  • Be the primary link with the social prescribing team, to help co-ordinate the care for patients where non-medical issues may be impacting on their health and well-being, and thus access person-centred local support and education.
  • Have the flexibility to be able to cover reception when required during times of annual leave or sick leave
Receptionist / Administrator duties

  • Record and maintain accurate patient information, summarising and coding information onto the patients notes using the practice computer system.
  • Check, scan and electronically file correspondence from the local hospitals and community services
  • Coordinate and organise the Patient Participation Group; liaising with the group members and ensuring the agenda and minutes are completed.

We are a small, close knit practice serving around 4,500 patients, with a clinical team consisting of two General Practitioners, two part time General Practitioners, two Advanced Nurse Practitioners, one Practice Nurse, one Healthcare Assistant, and a GP Assistant.

Job description

Job responsibilities

The following are the core responsibilities of the care coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Maintaining and monitoring the practice appointment system.
  • Process personal, telephone and e-requests for appointments.
  • Answer incoming phone calls, transferring calls or dealing with the callers request appropriately.
  • Signpost patients to the correct service.
  • Initiating contact with and responding to, requests from patients, team members and external agencies.
  • Be the primary link with the social prescribing team, to help co-ordinate the care for patients where non-medical issues may be impacting on their health and well-being, and thus access person-centred local support and education.
  • Coordinate and organise the Patient Participation Group; liaising with the group members and ensuring the agenda and minutes are completed.
  • Process and distribute incoming and outgoing mail.
  • Action incoming emails when necessary.
  • Complete opening and closing procedures in accordance with the duty rota and maintaining security in accordance with Practice protocols.
  • As required support the management of repeat prescriptions, ensuring they are processed accurately and efficiently in accordance with practice guidelines.
  • Photocopy documentation as required
  • Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the administrative team.
  • Clearing and re-stock consulting rooms as required.
  • Manage all queries as necessary in an efficient manner.
  • Maintain a clean, tidy, effective working area at all times.
  • Monitor and maintain the reception area and notice boards, keeping the reception area free from clutter and obstructions.
  • Support all clinical staff with general tasks as requested
  • Provision of refreshments for staff and visitors as required.

In addition to the primary responsibilities, the medical administrator may be requested to:

  • Support administrative staff, providing cover during staff absences.
  • Scanning of patient related documentation and attaching scanned documents to patients healthcare records
  • Read code data on EMIS clinical system.
  • Action tasks from the clinical team in a timely manner.
  • Data entry of new and temporary registrations and relevant patient information as required
  • Input data into the patients healthcare records as necessary
  • Carry out system searches as requested
  • Ordering and monitoring of stationery supplies
