Kearsley
Medical Centre is seeking to appoint a highly skilled, motivated and
patient focused First Contact Practitioner FCP Musculoskeletal Practitioner
to deliver high quality, evidence based MSK care within Primary Care. The
postholder will work primarily from Kearsley Medical Centre, with agreed
hosted sessions delivered at Fig Tree Medical Centre, Farnworth.

Main duties of the job
This
post is funded via the Additional Roles Reimbursement Scheme ARRS and is
entirely dependent on continued eligibility and reimbursement under national
ARRS guidance and local ICB approval.
To work as an autonomous First Contact Practitioner within Primary Care,
providing specialist MSK assessment and management for patients presenting with
undifferentiated MSK conditions without prior GP assessment. The postholder
will deliver rapid diagnosis, treatment planning and onward referral where
required, acting as an expert MSK resource to the Practice team.
The postholder will work in accordance with Practice clinical governance
arrangements, agreed scope of practice, escalation pathways and GP clinical
oversight where required, and in line with NHS England FCP and ARRS frameworks.
This is a Primary Care role focused on undifferentiated first contact MSK
presentations.

Kearsley Medical Centre is based in South Bolton, close to the
A666 and provide care to approximately 13,500 patients. We are a training
practice and support trainee GPs and as other professional placements, as well
as our employed staff in their career development.
We are proud to have a friendly team of circa 50 members of
staff, including PCN staff members, who all work collaboratively to deliver the
best care to the patients in our community.
Our clinical team consists of 6 GP Partners, 2 Salaried GPs, 2
ACPs, 3 Practice Nurses, 1 Nursing Assistant Practitioner, 1 PCN Paramedic, MSK
and Mental Health Practitioners, and a Pharmacy team.
Our clinicians are supported by reception, admin, clinical admin
and management teams.

Cross Practice Working - Hosted Sessions at Fig Tree Medical Centre
- Deliver up to 2 MSK clinical sessions per week at Fig Tree Medical Centre
under a formal hosting subcontracting arrangement between practices.
- Remain at all times an employee of Kearsley Medical Centre, including when
delivering care at Fig Tree Medical Centre.

- Maintain full compliance with information governance, safeguarding and
incident reporting processes at both sites.
Clinical Practice
- Work autonomously within agreed scope of practice, managing own specialist
MSK caseload.
- Undertake comprehensive MSK assessment of patients with acute, chronic and
complex conditions.
- Triage, diagnose, plan, implement and evaluate individualised management
plans.
- Request, interpret and act upon relevant investigations e.g. blood tests,
imaging in line with agreed protocols.
- Deliver evidence-based management aiming to optimise function, independence
and self-management.
- Provide supported self-management programmes including exercise prescription
and education.
- Make timely, appropriate referrals to secondary care and community services.
- Deliver urgent MSK triage and admission avoidance.
Advanced Practice Skills
- Where appropriately qualified and credentialed, undertake nonmedical
prescribing and joint/soft tissue injections in accordance with local policies,
PSDs or PGDs, agreed scope of practice, supervision arrangements and professional
indemnity requirements.
Clinical Governance, Quality & Safety
- Maintain accurate, contemporaneous clinical records on EMIS or SystmOne.
- Participate in clinical audit, quality improvement activity and service
evaluation.
- Contribute to incident reporting and learning processes.
- Comply with GDPR, Caldicott principles and all Information Governance
requirements.
- Adhere at all times to safeguarding policies and procedures.
Education & Service Development
- Act as an MSK clinical resource for the multidisciplinary team including GPs,
nurses and allied health professionals.
- Contribute to education and training of staff where appropriate.
- Support service development, pathway redesign and demand management
initiatives at Practice and PCN level.
Professional Responsibilities
- Participate in regular clinical supervision, appraisal and mandatory
training.
- Maintain HCPC registration, CPD portfolio and revalidation where applicable.
- Maintain appropriate professional indemnity at all times.

- The postholder will remain under the clinical governance of Kearsley Medical
Centre at all times, including when delivering hosted sessions at Fig Tree
Medical Centre.
- Named GP clinical supervision will be provided by Kearsley Medical Centre.
- Peer MSK supervision will be supported as required.
- Clear escalation pathways will operate across both sites.

The duties of this role may be reviewed and amended in accordance with the
evolving needs of the service following consultation with the postholder. The
postholder may be required to deliver agreed sessions across more than one
practice site in line with hosting/subcontracted service delivery
arrangements.
