Job summary

St Luke's Surgery is seeking a Practice Nurse to join and complement our working family team.We are looking for a practice nurse to join our clinical team and support our GP's, 3 days per week. We are looking for a nurse that is confident and forward thinking. All applicants must hold a NMC pin number.

Main duties of the job

The main duties will be to undertake regular clinics within the practice. To be able to maintain our long term condition registers and have a good knowledge and understanding of infection control. To become our infection control lead.Cervical screenings, childhood immunisation, travel vaccines. Also to clinically support our doctors.

Job description

Job responsibilities

Deliver and oversee a range of clinical services, including chronic disease management, health promotion, immunisations, and wound care.

To work as part of the Practice's multidisciplinary team, the post holder will effectively deliver nursing services to the patient population.

To provide routine nursing care to patients as required in accordance with clinical based evidence.

To provide wound care services to patients.

To provide travel advice and vaccines where necessary.

To request pathology services, as necessary and process pathology results, as required.

To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.

To maintain accurate clinical records in conjunction with extant legislation.

To ensure effective use of read codes, understand and complete QOF work.

To chaperone patients when needed.

To support the Team in dealing with clinical emergencies if needed

To implement vaccination programmes for adults and children.

To order vaccinations from suppliers

To have a positive attitude, to be committed to the role and work as part of a team.

Be punctual and reliable.

Ability to multi-task and work under pressure.

To be motivated, keen to learn and keep up to date.

Excellent communication skills and telephone manner.

Commitment to ongoing professional development and evidence of CPD.

Women's health - cervical cytology, contraception, HRT etc.

Good working knowledge around hypertension and CHD.

Immunisations (routine, childhood and travel)

Understanding of the cold chain and what to do if there is a breach.

Wound care skills leg assessments, dressing techniques, leg ulcer management. Be aware of who and when to refer to other services.

Requesting pathology results, able to interpret results and feedback to patients.

Understand the importance of evidence-based practice

Ability to record accurate clinical notes

Ability to work within own scope of practice and understand when to refer to GPs

Good understanding of infection prevention and control measures.

Understand the requirement for PGDs and associated policy

Strong IT skills, able to use Emis and AccuRx

Effective time management (planning and organising)

Ability to follow clinical policy and procedure
