Job summary
The Hadwen & Quedgeley Primary Care Network (HQ PCN), comprising of Hadwen Health and Quedgeley Medical Centre, is looking expand its clinical pharmacy team by recruiting an additional Clinical Pharmacist to work at its Hadwen Health surgery. We are looking for an individual who is passionate in delivering high quality service to patients. The successful candidate will be an enthusiastic and cheerful individual with a high degree of personal credibility, emotional intelligence, patience, and flexibility.
Main duties of the job
Clinical Pharmacists work as part of general practice teams to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
Having Clinical Pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.
About us
Based in the new, modern Hadwen Health building, this role will work as part of the PCN and Practice team, made up of GPs, Advanced Practitioners, Clinical Pharmacists, Pharmacy Technicians, Care Coordinators, Social Prescribers, Mental Health Practitioners and Physiotherapists. The PCN, overseen by a GP Clinical Director and Business Manager, offers opportunity for practitioners to work with a wide demographic of patients and to see a broad range of conditions and health inequalities, as well as training opportunities such as the FCP Roadmap.
Hadwen Health was founded in 1896 by Dr Walter Hadwen; a qualified pharmacist who went on to train as a mature student at Bristol University Medical School, winning Gold Medals in Medicine and Surgery. Job responsibilities
Management of medicines at discharge from hospital
Reconcile medicines following discharge from hospital, intermediate care and into care homes; identify and rectify unexplained changes; perform a clinical medication review; produce a post-discharge medicines care plan including dose titration and booking of follow-up tests and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge
Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g., those with medicine compliance aids or those in care homes)
Risk stratification
Design, development, and implementation of computer searches to identify cohorts of patients at high risk of harm from medicines
Responsibility for management of risk stratification tools on behalf of the Practice
Working with patients and the primary care team to minimise risks through medicines optimisation
Unplanned hospital admissions
Review the use of medicines most associated with unplanned admission and readmissions from medicines
Put in place changes to monitor and review the prescribing of these medicines to high-risk patient groups
Repeat prescribing
Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates; make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required
Telephone and patient facing medicines support
Provide a telephone help line for patients with questions and concerns about their medicines
Medication review
Hold clinics for patients requiring face-to-face and telephone Structured Medication Reviews. I.e., a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine taking.
Implement own prescribing changes (if an independent prescriber) and order relevant monitoring tests
Care home medication reviews
Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy
Work with care home staff to improve safety of medicines ordering and administration
Long-term condition clinics
Run own long-term condition clinics where responsible for prescribing as an independent prescriber for conditions where medicines have a large component (e.g., medicine optimisation for hypertension and asthma)
Service development
Provide advice on the development of new services and NICE guidance that have a medicine component
Care Quality Commission
Provide leadership to the Partners to ensure the Practice is compliant with CQC standards where medicines are involved

Support public health campaigns
