Job summary

We are seeking and enthusiastic and motivated Practice Nurse to join our existing high performing nursing team. Ideally the successful candidate will have experience of respiratory disease monitoring.

Main duties of the job

Duties will include general nursing, chronic disease monitoring, travel vaccinations, childhood immunisations, family planning, cervical screening, and treatment of minor injuries. Previous experience in some of these areas is essential. Candidates should be adaptable and able to work efficiently in a busy Primary Care environment.

The post is ideally for 22.5 hours per week with the successful candidate working Mondays OR Tuesdays, Thursdays and Fridays

We are a training Practice with 4 Partners and 8 salaried doctors, and offer a supportive, well-managed working environment and a happy and functional Practice team. Our nursing team is made of 6 practice nurses, with 1 about to retire. We offer a generous holiday allowance and membership of the NHS Pension Scheme. We have moved into a new, purpose built building that will help us meet the future clinical needs of our patient population as well as providing new office accommodation for our administration, IT, and patient services teams.

Job summary

The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.

Assess, plan, develop, implement and evaluate treatment programmes that promote well being
