Portsmouth North Primary Care Network are seeking a qualified Prescribing Pharmacist to join our rapidly expanding existing team serving patients across our area consisting of The Drayton Surgery and Kirklands Surgery.
The successful candidate will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
They will support the Lead Pharmacist in developing more junior team members and assist with audits, process reviews & reporting.
Please note, The Drayton Surgery is the employing practice for Portsmouth North PCN and the role is based across the PCN footprint.
Main duties of the job
The successful candidate should be an independent prescriber and have the necessary qualifications from the necessary pathways. We are willing to consider someone who is at the end of their IP pathway.
The post holder will provide primary support to general practice staff with regards to prescription & medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice, while addressing both public and social care needs of patient in the GP practice(s).
The post holder will provide clinical leadership on medicines management and quality improvement as well as managing some aspects of the quality and outcomes framework and enhanced services.

Working under our Lead Clinical Pharmacist you will work across the PCN footprint, providing high level support to our team of GPs and clinicians.
Our 2 member practices are located on the northern and eastern edges of Portsmouth City. We have a combined patient list size of approximately 37k patients, along with a full range of both clinical and administrative support staff.
