Clinical

Provide patient centred comprehensive consultations in a routine and same day capacity at both MMC and our branch surgery in Earls Barton. To be undertaken via face to face, telephone or video consultation. Deliver chronic disease management in line with current local practice, NICE guidelines and where appropriate Abbey Medical Practice guidelines Deliver a fully holistic, patient centred focus being mindful of the wider MDT and other organisations available to work in conjunction with. Refer patients where appropriate, within the accepted timeframes for 2ww and urgent referrals and all other routine referrals. Escalate safeguarding concerns to the Practice Safeguarding Lead in accordance with Practice Procedures. Keep high quality, comprehensive and accurate records for all contacts in patient records. Ensure patient records are read coded appropriately. Ensure tasks, signing prescriptions, actioning referrals, scanned letters, blood results, completing reports and all other administrative tasks are appropriately actioned with timely manner Prescribe medication in accordance with Practice/ICB Prescribing formulary/incentive scheme. Undertake medication reviews within timescales annotated on patient record. Carry out home visits as assigned to you daily, either in response to urgent requests or reviewing patients. Update patient records upon returning to the practice Adhere to the Practice Infection Control policy and procedures are all times. Keeping own working areas clean, sterile and free of hazards, risks. Escalate where appropriate. Ensure you are available to start the on call from 8am (when this is a morning session) and until 6.30pm (when this is an afternoon session) unless prior agreement has been authorised which varies from these times. Participate in the practice extended hours on a pro-rata basis.

Training and Personal Development

Willingness to undertake professional development Participation in clinical meetings, Protected Learning Time and adhoc training sessions Ensure that all training is up to date, including mandatory Abbey Medical Practice training and statutory GP training (for example: safe guarding, Basic life support and anaphylaxis) Complete annual in house appraisal, as well as statutory appraisal. Complete one audit per year Reflect on own performance, learning and take accountability. Share best practice with colleagues through debrief sessions, significant events and complaints process. Assist with mentoring of medical students and registrars as directed.

Suggestion for Change

To be willing to adapt current working procedures in response to changes directed by the Partners within Abbey Medical Practice or in response to external changes, such as General Practice Forward View, Northamptonshire ICB or CQC requirements. Always look at ways the Practice can improve and provide suggestions that will improve processes and ultimately patient care.

Learning and Development

The effective use of training and development is fundamental in ensure that all staff are equipped with the appropriate skills, knowledge, attitude and competencies to perform their role. All staff will be required to partake in and complete mandatory training as directed by the Compliance Manager as well as participating in practice training programmes. Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately enable them to improve processes and service delivery.

Service Delivery

Staff at Abbey Medical Practice, must adhere to the information contained within practice policies and regional directives, ensuring protocols are followed at all times. Staff will be given detailed information during the induction process regarding policy and procedure.
