Job summary

A Clinical Practitioner at The Pembroke Medical Group will be an experienced clinician who, acting within their professional boundaries, will provide care for patients presenting at the practice from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, autonomous clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.

Main duties of the job

Ø Assess patients presenting with acute illness

Ø Undertake home visits for acute illness/injury/underlying chronic disease

Ø Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

Ø Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Ø Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs.

Ø Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Ø Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Ø Fully participate in the clinical administration within the practice, signing prescriptions, reviewing medication, EPS, reviewing patient letters/discharges, and respond to queries.

Ø Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.

Ø Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.

  • Minimum 5 years post registration experience
  • Leadership in quality initiatives such as clinical benchmarking
  • Extended and supplementary nurse prescribing.
  • Nurse-led management of minor illness, minor ailments and injuries including children

  • Recent primary and community nursing experience
  • Nurse-led triage including telephone triage
  • Working with community development initiatives
  • Health-needs assessment of patients
