We are
seeking a Patient Care Coordinator (PCC) to join our dedicated and committed team focusing on the
provision of a quality service to our patient population.
Working closely
with patients and clinicians, or other healthcare professional, the PCC
co-ordinates patients healthcare and directs them to the appropriate service
to ensure they get the most suitable care from whatever health or social care
provider is appropriate.
The PCCs role requires them to be able to work with,
and understand the roles of, a variety of different people working in the
practice and across the PCN including Doctors, Nurses, Healthcare Assistants,
Social Prescribing Link Workers, Physiotherapists, Physician Associates,
Paramedics, Health and Wellbeing Coaches, Podiatrists, Occupational Therapists
and Pharmacy Technicians.
Main duties of the job
The main duties of the role include:
- Supporting patients
to utilise decision aids in preparation for a shared decision-making
conversation
- Helping people to
manage their needs through answering queries, making and managing appointments
and ensuring that people have good quality written or verbal information to
help them to make choices about their care, using tools to understand peoples
level of knowledge and confidence in skills in managing their own health

Providing coordination and navigation for people and their carers across health and care
services, working closely with Social Prescribing Link Workers, Health and
Wellbeing Coaches and other primary care professionals

Liaising with
relevant members of the multidisciplinary team and utilise, where appropriate,
other services available such as Pharmacy First, dependent on patient needs.
The role is a new role and evolving post that will adapt and change according to the developing needs of the team and applicants should be prepared for that.
About us
The working pattern is
approximately 25 to 28 hours per week, a longer working day on Monday, with a
daily start time of 8:30/8:45am.
Starting
rate of pay is currently £11.50 p/hr but is currently under review and will
increase following 1st April 2025. Benefits include 6.6 weeks holiday inclusive
of bank holidays pro rata, access to the NHS pension scheme, occupational sick pay (after
qualifying period), a uniform
and allocated parking (as part of a rota).
