Job summary
Due to a growing patient list, we have an exciting opportunity for a PCN Advanced Clinical Practitioner (Nurse Practitioner or Paramedic) to join Bramblehaies team, working three days week supporting our same day duty service.
Our same day duty service is currently covered by a Duty Doctor and Nurse
Practitioner and supported by well-trained care navigation and administration teams.
The Advanced Clinical Practitioner will share the duty list with the duty GP,
providing a service to our patients presenting with acute conditions.
Applicants must be NMC or HCPC registered, qualified as an Advanced Practitioner and be an Independent Prescriber. If a registered nurse they must be educated to master's level or equivalent that encompasses the four pillars of clinical practice with demonstration of core capabilities and area-specific clinical competencies.

Main duties of the job
The post holder will be an integral part of our General
Practice multi-disciplinary team, working within their scope of practice using
their enhanced clinical assessment and treatment skills to assess and manage
patients presenting with acute presentations as part of the on the day duty
service team.
About us
Bramblehaies Surgery, in Cullompton, is a PMS training practice in the
heart of Devon yet conveniently situated 3 minutes off Junction 28 of the M5. We
deliver a high standard of care to our 7,400-patient list in the town and
surrounding villages from a partner-owned modern, purpose-built building which
has recently been extended to provide further consulting rooms.
Our team currently consists of four GP Partners, two salaried GPs, Practice Manager, Patient Services Manager, Data Quality Administrator, Clinical Pharmacist and Pharmacy Technician, a team of Practice Nurses headed by a Lead Nurse, three HCAs, a GP Assistant, a committed admin/reception team and our much-needed housekeeping team.
We continue to rise to the challenges experienced from working in such a
dynamic environment, maintaining our enthusiasm for delivery of care and retaining
our sense of humour.
We are a member practice of Culm Valley Primary Care Network and are well
supported with Social Prescribers for Adults and Young People, a Health and Wellbeing
Coach, First Contact Physiotherapist, Mental Health Worker and the Community
Healthcare Team. Our Care Home Team consists of a Nurse Practitioner and an
Occupational Therapist.
Our clinical system is SystmOne; we offer 15-minute appointments, a
daily clinical huddle which provides an opportunity discuss clinical matters as
a multidisciplinary team to provide support
for our entire team.

a.Work as part of a multi-disciplinary team (MDT) within Bramblehaies Surgery and as part of the wider PCN clinical team.
b.Assess and triage patients including same day triage and, as appropriate, provide definitive treatment (including prescribing medications, following policy, patient group directives, national e.g. NICE and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
c.Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
d.Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
e.Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.
f.Be able to perform specialist health checks and reviews within their scope of practice and in line with local and national guidance;
g.Support the delivery of anticipatory care plans and provide an alternative model to urgent and same day GP home visit for the Practice/PCN.
h.Complete the relevant training to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example Practice Nurses and the personalised care roles.
i.Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service and, where appropriate, other community or voluntary services.
j.Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
k.Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
l.Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
m.Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices.
