Are you an enthusiastic and compassionate Practice Nurse looking to
make a difference in primary care?

seeking a dedicated and experienced nurse to join our dynamic and
patient-focused team at Bramblehaies Surgery, working two to three days a week,
hours negotiable. We also provide a
service one Saturday each month.

Main duties of the job
As our
Practice Nurse you'll play a key role in delivering high quality nursing care
and health promotion to our diverse patient population. You'll work alongside a
multidisciplinary team to support chronic disease management, immunisations,
cervical screening, wound care, health promotion and more.

Bramblehaies
Surgery, in Cullompton, is a semi-rural training practice in the heart of Devon
serving a population of 7668 patients, providing high standards of care (CQC
rating of Good) from a modern, purpose-built building.
Our
team currently consists of four GP partners, two salaried GPs, Practice
Manager, Patient Services Manager, Data Quality & Systems Administrator, a
Pharmacy team, Nurse Practitioners, a team of Practice
Nurses and HCAs, a committed admin/reception team and our much-needed
housekeeping team. Our patient list is growing so no doubt our staff team will
continue to grow to meet the needs of the population we provide care to.

Assess, plan, develop, implement and evaluation treatment programmes that promote health and well-being, and also patients with a known long-term condition
Work with other health care professionals to monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols with support from NMP or GP
Work with patients in order to support adherence to prescribed treatments
Support and manage health needs of women presenting for cervical cytology consultations
Implement and participate in vaccination and immunisation programmes for both adults and children
Advise, support and administer vaccinations where appropriate for patients travelling abroad
Provide wound care support when necessary
