Job summary

Okehampton Medical Centre is looking for an experienced health care professional who can join our forward thinking same day team.

Main duties of the job

The same day team is made up of GPs, nurse practitioners, pharmacist and paramedics; the role will consist of phone triage, home visits and working in the minor injuries unit.

An independent prescribing qualification is recommended but support with obtaining this qualification can be discussed.

This will be a part time post 2/3 days weekly.

Job responsibilities

Job Purpose

  • To deliver a high standard of patient care within your scope of practice to meet the needs of the Practice patient population
  • To deal with presenting patients needs in a Primary Care setting
  • To provide clinical leadership and support to other members of the team
  • To work in and support the Minor Injury Unit

Main Responsibilities

Clinical:

  • Provide first point of contact for patients presenting with undiagnosed problems, using history taking, physical examination, problem solving and clinical decision making skills to reach a working diagnosis and management plan in partnership with patients and their carers
  • Undertake Practice based consultations for emergency or routine problems including management of exacerbations of long term conditions
  • Undertake telephone consultations, clinics, home visits and participate in our same day service during Practice opening hours
  • Instigate diagnostic tests or investigations in Primary Care and interpret findings/reports within the scope of your practice, discussing the results and implications with your patients
  • Utilise clinical guidelines and promote evidence based practice
  • Refer patients directly to other services using appropriate referral pathways
  • Provide preventative healthcare and advice
  • Provide medical support as required to patients
  • Ensure safe handover of care within and outside the Practice as appropriate
  • Work directly with members of the Practice team and support integrated patient centred care working with wider Primary Care / Social Care networks
  • Identify Community Health needs and participate in the development of patient/family centred strategies to address them

  • Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements

  • Apply infection control measures within the practice according to local and national guidelines
