Job summary

This is an exciting opportunity to join a new team providing a new model of care across Plymouth, through the establishment of the Inclusion Health Service.

In line with current best practice this service is being developed using the “Pathway” approach, providing ‘end to end’ support for individuals who are homeless. It involves not only medical staff, but a range of multidisciplinary professionals with expertise in social care, housing law and benefits issues, ensuring that a person’s full range of needs are supported. The aim of the Inclusion Health service is:

  • To create a comprehensive service, for adults with complex needs related to homelessness, health, contact with the criminal justice system and substance misuse issues.

The Service will:

  • Prevent more entrenched and longer-term homelessness.
  • Contribute towards improving outcomes and addressing health inequalities for some of the most disadvantaged individuals in Plymouth.
  • Achieve truly joined up and effective care and ensure engagement and completion of treatment wherever possible.

    Main duties of the job

    The post holder be an experienced mental health nurse who  will provide care for service users from initial referral, clinical assessment, needs assessment, intervention through to safe discharge.

    A key aim will be to engage clients and staff to work collaboratively with the team to address homelessness.  The post holder will work in partnership with other members of the multi- disciplinary team, as well as other agencies to achieve successful outcomes. The post holder will have expert knowledge of health, social and housing services within the community.

    The post holder will be a resource to others about mental health conditions in the context of homelessness. The post holder will be expected to have in-depth knowledge of the Mental Health and Mental Capacity Acts.

    All team members are expected to work collaboratively and supportively with each other across Primary, Secondary Care and Community care boundaries to improve outcomes for people who are homeless.

    They should also work to the Pathway model (with flexibility as required), to attend team handover and MDT meetings, and to be present to take referrals, and undertake assessments and ward rounds where needed.

    The Plymouth Alliance was formed with a group of local charities / community organisations with leadership from Plymouth City Council to provide services to people with complex needs (homelessness, drug or alcohol misuse, mental illness and offending).

    Our Plymouth health and care system requires and features strong and effective collaboration.

    This role will be employed by one of these partner organisations.

    Clinical

    • To work autonomously, managing a caseload of patients, whilst working as part of the multidisciplinary team.
    • To ensure that the team assess, diagnose, plan, implement and evaluate effective health and social interventions for inpatients experiencing homelessness, and to be available to support and advise on the most complex and high-risk cases.
    • To deliver individualised, highly specialised advice to the client group, liaising with necessary support agencies/authorities to achieve the best possible outcome on discharge.

    • Act as an advocate for service users in a number of settings, with an overall aim to gain positive outcomes in housing and onward mental health/physical health support and care.
    • Establish effective communication and actively promote health, housing and welfare rights education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment.
    • Liaise and communicate effectively with multidisciplinary and primary care teams across health, social care and housing boundaries, ensuring timely access to community services.
