Job summary

This is an exciting opportunity to provide a new model of care across Plymouth, through the establishment of the Inclusion Health Service.

In line with current best practice this service is being developed using the “Pathway” approach, providing ‘end to end’ support for individuals who are homeless. It involves not only medical staff, but a range of multidisciplinary professionals with expertise in social care, housing law and benefits issues, ensuring that a person’s full range of needs are supported. The aim of the Inclusion Health service is:

  • To create a comprehensive service, for adults with complex needs related to homelessness, health, contact with the criminal justice system and substance misuse issues.

The Service will:

  • Prevent more entrenched and longer-term homelessness.
  • Contribute towards improving outcomes and addressing health inequalities for some of the most disadvantaged individuals in Plymouth.
  • Achieve truly joined up and effective care for this most vulnerable group of individuals and ensure engagement and completion of treatment wherever possible.

Main duties of the job

  • To develop and oversee the provision of an evidence-based occupational therapy service for adults with complex needs related to homelessness, health, contact with the criminal justice system and substance misuse issues.
  • To provide highly specialist occupational therapy assessment, formulation, and interventions to adults with high levels of complexity and vulnerabilities.
  • To provide specialist advice, supervision, case discussion & formulation to staff supporting adults who have a diagnosis of a severe and enduring mental health issue.
  • To provide specialist occupational therapy input to planning, development and evaluation of clinical services within the mental health services, holding responsibility for defined projects.
  • To support robust risk assessment and management, offering specialist plans and intervention which support individuals within the community setting.
  • To promote inclusion, awareness of occupational performance issues and onward support.
  • All team members are expected to work collaboratively and supportively with each other across Primary, Secondary Care and Community care boundaries to improve outcomes for people who are homeless.
  • They should also work to the Pathway model (with flexibility as required), to attend team handover and MDT meetings, and to be present to take referrals, and undertake assessments and ward rounds where needed.

The Plymouth Alliance was formed with a group of local charities / community organisations with leadership from Plymouth City Council to provide services to people with complex needs (homelessness, drug or alcohol misuse, mental illness and offending).

Our Plymouth health and care system requires and features strong and effective collaboration.

This role will be employed by one of these partner organisations.

Leadership

  • To ensure patient/group treatment programs are monitored, evaluated and modified as required on a regular and systematic basis to measure progress and ensure effectiveness of intervention.
