Job summary
We are looking to recruit a Paramedic Practitioner who will be an integral part of The Jubilee team, primarily working with our
multi-disciplinary team across all of our workstreams with a particular
emphasis of supporting our delivery of in-hours urgent care.
The
successful candidate will be an experienced and enthusiastic clinician, who is
passionate about developing a career in Primary Care where they will have the
opportunity to directly influence patient care, applying a holistic and
preventative medicine approach in addition to responding to acute medical need.

Main duties of the job

Be the first point of contact for acute minor
illness presentations utilising diagnostic skills in providing safe management
plans

Supporting GPs with managing results
- Supporting GPs with their paperwork
- Review, analyse
and action diagnostic test results.
- Home visits and duty rotas
- Deliver integrated patient
centred-care through appropriate working with the wider primary care
multi-disciplinary team and social care networks.
- Health/disease promotion and
prevention advice to patients.
- Promote evidence-based practice and partake in clinical audits, SEA reviews
- Undertake face to face, online, telephone consultations and
ensure care is fully documented

support and continue the development of an on the day emergency patient
service in the Practice.

well-established team of 4 GP partners, salaried
GPs and Allied Healthcare Professionals. We
are part of Melton, Syston and Vale Primary Care Network (PCN) which covers a
total population of circa 69,500 patients. Of this, The Jubilee Medical
Practice covers a mixed demographic population of circa 12,000 patients.
We
are a high achieving practice and we were rated Good at our most recent CQC

in academia and research at a local and national level makes us a perfect

provide assessment, diagnosis and treatment at first point of contact by
attending to patients according to patients needs either in the Practice
or patients homes.
- To
support and continue the development of an on the day emergency patient
service in the Practice.
- To
accurately triage and prioritise patients, including in emergency
situations, demonstrating the use of a variety of techniques to elicit the
history of an event/illness, including past medical and drug history.
- To
evaluate clinical information from examination and history taking, and
initiate appropriate treatment and/or referral.
- To
undertake and interpret diagnostics and take appropriate action to manage
patient care, whilst distinguishing between abnormal and normal findings
in order to relate them to a possible diagnosis. To use broad clinical
guidelines (NICE etc) relating them to individual cases, providing
assistance and feedback to the organisation and others on how these should
be interpreted.

refer patients to appropriate care settings, within locally agreed

Qualifications
Essential
- Educated to degree/diploma level in Paramedicine or
- equivalent experience
- Registered with the Health and Care Professions Council
- (HCPC)
- Completed their two-year Consolidation of Learning period
- as a newly qualified paramedic.
- Has a further three years experience as a band 6 (or equivalent
- paramedic)
- Able to demonstrate Level 7 capability
