We are Affinity Care PCN, and we have an excellent opportunity to join a dynamic and energised organisation focused on delivering the highest quality care to our patients, working as our Vaccination and Immunisation Programme Lead.

We are made up of 4 practices and 8 sites across Bradford, covering a number of locations from Denholme to Greengates.

Our Vaccination & Immunisation Programme Lead manages the day-to-day running of our vaccination and immunisation activity.

This is a vital function involving planning and co-ordinating all vaccination clinics in line with national guidance and timelines, aimed at the successful implementation of vaccination programmes for all of our patients across all age groups and all of our sites, including our patients who are resident in care homes or housebound.

The postholder will also be responsible for ordering of stock and ensuring stock levels are monitored and sufficient to deliver the care our patients need.

Our ideal candidate must have experience working in health and/or social care or relevant public sector experience, experience of forward planning of service activity, and be able to use SystmOne or a similar clinical system.

Main duties of the job

Handle enquiries from the general public around scheduled vaccination and immunisation programmes including but not limited to Childhood Immunisations, Seasonal Influenza and Covid-19.

Proactively follow up with patients to book in their vaccinations in line with eligibility criteria as defined by the NHS.

See things from a patient perspective; identifying ways to improve our systems to enhance the patient experience.

Support the administrative and vaccination teams to provide a quality system of work that is effective in its use of team resources.

Review, respond to and disseminate external vaccination related emails and communications

Escalate all relevant issues and information to Patient Services Manager for Planned Care, ensuring they are aware of any developments impacting the operational functioning of the team.

Use the relevant ordering systems to ensure appropriate levels of stock are available against predicted demand/agreed activity

Work with other colleagues to ensure stock management and data returns are completed accurately and in a timely manner

Support the collation of all activity data and reporting information as required.

Job responsibilities

The post is part of the Affinity Care Primary Care Network Team and will provide day to day support in the bookings of appointments for patients linked to the roll out of our vaccinations (vaccs) and immunisation (imms) programmes including the current COVID Vaccination programme, the influenza programme, Shingles, baby vaccs and imms, other adult immunisation programmes such as HPV, Pertussis in pregnancy, Meningitis ACWY and any catch-up programmes as introduced from time to time.

Handle enquiries from the general public around scheduled vaccination and immunisation programmes including but not limited to Childhood Immunisations, Seasonal Influenza and Covid-19.

Proactively follow up with patients to book in their vaccinations in line with eligibility criteria as defined by the NHS.

See things from a patient perspective; identifying ways to improve our systems to enhance the patient experience.

Support the administrative and vaccination teams to provide a quality system of work that is effective in its use of team resources.

Plan and co-ordinate all vaccination clinics in line with national timelines and agreed resources, working with the relevant teams i.e. Nursing, Complex Care At Home Teams.

Support the Patient Services Manager for Planned Care in the development and implementation of successful seasonal vaccination programmes across all sites, care homes and housebound

Review, respond to and disseminate external vaccination related emails and communications

Be the link to the external co-ordinators for vaccinations and immunisations

Maximise the use of new technology and methods of communication with patients

Apply proven and innovative approaches to encourage attendance and reduce DNA.

See things from a patient perspective; identifying ways to improve our systems to enhance the patient experience, attendance at clinics and successful vaccination

Escalate all relevant issues and information to Patient Services Manager for Planned Care, ensuring they are aware of any developments impacting the operational functioning of the team.

Use the relevant ordering systems to ensure appropriate levels of stock are available against predicted demand/agreed activity

Work with other colleagues to ensure stock management and data returns are completed accurately and in a timely manner

Support the collation of all activity data and reporting information as required.

Use clinical systems to review current performance and identify areas and locations for improvement in delivering vaccinations and immunisations to the required national standards

Constantly seek service improvement, mindful of the patient experience and patient safety balanced with efficient use of our resources.

Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

Escalate any concerns regarding patient welfare or safeguarding to the relevant clinical colleagues and safeguarding leads

Understand and follow the requirements of the General Data Protection Regulations (GDPR) and to refer on any queries as appropriate.

Adhere to the Affinity Cares policies for data protection, maintaining confidentiality of information at all times

Take the necessary precautions when transmitting information.

Follow organisational policy regarding the use of email and computer tasks as the main internal method of non-verbal communication and the use of the intranet and the internet as the main source of internal and external information.
