Part time Advanced Nurse Practitioner
An opportunity has arisen in our small friendly practice for an Advanced Nurse Practitioner to join our team. This is an additional advanced nurse practitioner post allowing the successful applicant an opportunity to develop the position in line with practice policies and procedures.
The primary objectives will be to effectively manage the Practices same day appointments, telephone triage and seeing patients in own clinic.
Main duties of the job
The role requires the successful candidate to work autonomously, making clinical decisions based on experience, knowledge and analytical abilities. This will include treating undiagnosed presentations.
We are proud to be a training practice and might be prepared to mentor the right candidate to upskill if needed.

Job summary:
Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs. The nurse practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
Job responsibilities:
Diagnosing and treating patients presenting with minor illness
- Undertake telephone triage
- Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
- Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary
- Assist with the organisation and co-ordination of the provision of nursing services for the practice
- Provide nursing treatment to patients in participation with general practitioners using agreed protocols, including the taking of cervical smears
- Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary
- Competent and confident in managing people with long term health conditions
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with, and adherence to, prescribed
treatments.
Provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactions
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Implement and participate in vaccination and immunisation programmes as able
Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
Recognise and work within own competence and professional code of conduct as regulated by the NMC
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
Deliver care according evidence-based care guidelines
Maintain and control injectable and oral drug stocks in the treatment room and doctors
surgeries.
Manage/co-ordinate Pathological specimens and investigatory procedures: Undertake the collection of pathological specimens including intravenous blood samples, swabs, smears, etc.
Oversee the duties undertaken by the colleagues, as appropriate, and assist with training and mentoring as and when required.
Assist in the formulation of practice philosophy, strategy and policy.
Undertaking home visits.
Actively assist the Practice in achieving its Quality Outcomes Framework targets.
Make own referrals as appropriate.
Undertake telephone triage.
Administration and professional responsibilities
- Participate in the administrative and professional responsibilities of the practice team
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
- Ensure collection and maintenance of statistical information
- Attend and participate in practice meetings as required
- Restocking and maintenance of clinical areas and consulting rooms
Training and personal development
- Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.
- Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring CPD requirements are met.
- Develop and maintain a Personal Development Plan as part of professional portfolio
Liaison
- Work closely with other nurse team members, reception, administration, management and with other surgeries/organisations.
Meetings
- Attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.
