Job summary

We are seeking to recruit a qualified nurse prescriber to join our motivated, friendly and supportive GP Practice. The successful candidate will be confident in independently managing acute cases of minor illnesses arising from the clinical triage system. Seeing and reviewing patients with long term conditions. Supporting the triage and care navigation of patients as well providing care to patients face to face, over the telephone and sometimes in their own homes.

Main duties of the job

The successful candidate will be an experienced and motivated Advanced Nurse Practitioner/ Independent Nurse Prescriber with a strong Primary care background. Knowledge of chronic disease management, especially Asthma and COPD is highly desirable. They should have good IT skills, excellent organisational, communication and clinical skills and a good understanding of QOF, Enhanced Services and CQC guidelines especially in the area of infection control.

We have a diverse and complementary clinical team with novel roles for Physician's associates, Pharmacists, enmeshed within the traditional nurse and GP positions. The management and admin team is experienced and supportive.

  • Assist with the provision of nursing services for the practice.
  • Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary.
  • Be competent in the administration of childhood immunisation and travel vaccination. Be competent and confident in managing people with long-term health conditions
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with the evidence-based practice and national and practice protocols, and within the scope of practice
