Job summary

We are looking for a clinical pharmacist to join our practice team. Its an exciting opportunity to develop a pioneering role within primary care. The successful candidate will work independently and be a part of the practice team to deliver safe and effective use of medicines.

The Clinical Pharmacists is very much a patient-facing role and there will be plenty of opportunity to utilise your clinical skills to improve the health of the practice patients. This will include managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications.

Main duties of the job

The main responsibilities of the role include:

  • Providing advice in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice.
  • Providing medication review, and health education advice, to patients by face-to-face or telephone
  • Processing hospital medication discharge letters.
  • Supporting improvements in clinical care, through practice-based audits and implementing change.
  • take on responsibility for areas of chronic disease management, working proactively with other healthcare professionals across the organisation.

Friendly and supportive to our core, we are lucky to have a variety of skilled team members including First Contact Physiotherapist, Mental Health Nurse and Social Prescriber. We also have an advanced nurse practitioner for minor illness and an advanced nurse practitioner specialising in Diabetes who work alongside our practice nursing team to provide high quality care to our patients in Bushey and neighbouring area

Essential

  • Completion of Pharmacy degree and at least 3 years post-graduate experience.
  • Excellent clinical and medication knowledge.
  • Experience of undertaking medication reviews.
  • Registered with the General Pharmaceutical Council.
  • Will hold Personal Professional Indemnity Insurance.
  • Disclosure and Barring Service Check
