Clinical Responsibilities:

The Advanced Nurse Clinical Practitioner (ANP) works autonomously and is accountable for own professional actions.

• Undertakes Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing the PCN.
• Uses specialist knowledge and specific training to act autonomously in assessing, planning, and implementing a comprehensive management plan within their defined scope of practice.
• Elicit a patient history appropriate to the clinical situation, which will include, presenting complaint, history of the present illness, past medical history, social history, family history, medications, allergies, review of systems, risk factors and appropriate targeted history.
• Perform a physical examination tailored to the needs of the patients and the demands of the clinical situation.
• Assess and prioritise patients undergoing treatment.
• Demonstrate continual evaluation of the patients and use expertise to recommend adjustments/amendments to treatment plans, in consultation with the patient and members of the multi-professional team.
• Make direct referrals to other members of the health care team.
• Requests bloods and radiological test as appropriate and within agreed scope
• Request and perform diagnostic tests as agreed locally and within agreed scope.

• Assesses diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
• Assesses diagnoses and treats patients in surgery or own home who require acute medical attention and refer to appropriate agency.
• Refers patients as appropriate to other members of the multi-disciplinary team, secondary care and to other statutory and voluntary organisations.
• Interprets a range of diagnostic tests and routine clinical procedures.
• Makes critical judgements of the highest order to satisfy expectations and demands of the job.
• Works within professional guidelines and codes of conduct.
• Works with all relevant practice policies and procedural guidelines e.g., infection control, chaperoning, risk management etc.
• Extended and supplementary prescriber.
• Provides highly specialised advice concerning care normally beyond the scope of normal nursing practice e.g., establishes differential diagnoses, management/treatment plans and makes referrals as appropriate.
• Contribute to practice targets both local and national within the disease management agenda e.g., QOF/Prescribing Incentive Scheme.
• Audits outcomes of care against standards and initiate changes as necessary.
• To actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns about radicalisation.

The ANP will:

• Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
• Clinically examine and assess patients needs from a physiological and psychological perspective, and plan clinical care accordingly.
• Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
• Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice including national and local protocols within scope of practice.
• Meet the needs of patients presenting for opportunistic wound care.
• Undertake minor surgery as appropriate to competence.
• Critically appraise the impact that a range of social, economic, and environmental factors can have on health outcomes for individuals, and where applicable their family and carers.
• Initiate effective multi-disciplinary team activity as a lead member and understand the importance of effective team dynamics. This may include service delivery, research such as audit and quality improvement.
• Advocate for and promote person-centred working.
• Ensure safety netting advice is appropriate and the person understands when to seek urgent or routine review.
• Understand the complexities of working with people who have multiple health conditions physical, mental, and psychosocial.
• Simultaneously manage acute and chronic problems, including people with multiple morbidities and those who are frail.
• Safely prescribe and/or administer therapeutic medications, relevant and appropriate to scope of practice, including an applied understanding of pharmacology which considers physiological and/or pathophysiological changes and allergies.
• Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements.

Delivering a quality service

• Recognise and work within own competence and professional code of conduct as regulated by the NMC.
• Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures.
• Prioritise, organise, and manage own workload in a manner that maintains and promotes quality.
• Deliver care according to NSF, NICE guidelines, and evidence-based care.
• Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation.
• Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
• Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
• In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
• Evaluate patients response to health care provision and the effectiveness of care.
• Support and participate in shared learning across the practice and wider organisation
• Use a structured framework (e.g., root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents, and near-miss events.
• Assess the impact of policy implementation on care delivery.
• Working with the practice management monitor and develop the performance of the GPN team in accordance with local policies.
• Contribute to the appraisal of the performance of the team, providing feedback as appropriate.
• Understand and apply legal issues that support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients health procedures and local guidance.
• Working with practice management to ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Provide guidance and support to ensure appropriate referral if required.

Experience

Essential

• Significant post qualification experience (minimum of 4 years) in a relevant clinical field.
• Evidence of recent teaching, mentorship, and supervision of staff.
• Advanced skills in listening and information processing, alongside empathetic skills to ascertain, understand and respond to individuals complex needs and concerns.
• Advanced knowledge in managing patients with long term conditions.
• Knowledge of clinical governance, confidentiality, data protection and information governance in primary care.
• A solid understanding and experience of QOF.
• Knowledge of the local and national health policy and wider health economy.
• Experience of compiling protocols and clinical guidelines.

Desirable

• Knowledge of public health issues faced.

Qualifications

Essential

• Master's in advanced clinical practice or an equivalent MSc or NHS England Centre for Advancing Practice Digital Badge (or working towards it).
• Possession of 1st degree in Nursing at a minimum 2:2 classification
• 4 years experience as an advanced nurse practitioner, working at a masters level that encompasses the four pillars of clinical practice, leadership and management, education, and research.
• Possess a prescribing qualification.
• Active NMC Registration and license to practice.
• MSc Advanced Practice Programme or completed the Centres ePortfolio (supported) Route.
• Clinical supervision training and experience.
• Advanced Skills in managing long term conditions.

Desirable

• Recognised Mentorship Qualification.
• Quality Improvement/leadership training.
• Community nursing specialist qualification.
