Job summary

Canvey Primary Care Network are looking for a dedicated and experienced advanced nurse practitioner to join our team on a full time basis and contribute to our mission of delivering exceptional healthcare services to our community.

Based in our Canvey practices, you will play a pivotal role in delivering advanced clinical care to our patients. You will work autonomously, providing assessment, diagnosis, treatment, and management of a wide range of conditions, both acute and chronic.

Due to home visits the applicants will need to hold a valid full drivers licence.

Main duties of the job

  • Conduct comprehensive patient assessments, including history taking, physical examinations, and ordering diagnostic tests.
  • Diagnose and manage both acute and chronic conditions, making appropriate referrals where necessary.
  • Prescribe medications and treatments in accordance with current guidelines and best practices.
  • Provide expert advice and education to patients and their families regarding treatment options, preventative care, and self-management.
  • Collaborate with the multidisciplinary team to develop and implement care plans that meet the individual needs of patients.
  • Participate in the development of clinical protocols and pathways to improve service delivery.
  • Lead on quality improvement initiatives within the PCN.
  • Engage in professional development and training to maintain and enhance your clinical skills and knowledge.

  • Elicit a patient history appropriate to the clinical situation, which will include, presenting complaint, history of the present illness, past medical history, social history, family history, medications, allergies, review of systems, risk factors and appropriate targeted history.
