Job summary

The Coastal Medical Partnership Care Coordinator for Correspondence (CCC) position is an evolving role which encompasses providing efficient and comprehensive administrative coordination of all the incoming correspondence to CMP. This includes directing communications to the appropriate person in the organisation, as well as actioning some post. Of paramount importance is ensuring appropriateness and accuracy of information. The post holder will be expected to act as a central point for information and communication for the multi-disciplines within the practice.

The PCN Clinical Director oversees the role of the Care Co-ordinator and ensures they meet the criteria set out by NHSE/I in the PCN DES.

Main duties of the job

Key Responsibilities

To provide a full, comprehensive and efficient administrative coordination service to the clinical staff in accordance with practice policies and procedures to support all aspects of patient care. To act as a point of contact for patients and act as a focal point of confidential liaison between patients, doctors and the wider MDT.

To provide this level of administration you will need strong IT skills, the ability to work on your own initiative and to tight deadlines, be a proactive thinker and anticipate problems. You will be able to prioritise your own workload and assist the team with their workload.

Key Responsibilities:

To support clinicians and patients with the provision of support through communications received.

To coordinate the communication of healthcare professionals and non-clinical staff involved in the care of patients.

Ensure familiarity and adherence to all practice policies and protocols and ensure you have a practical awareness of what is required, including but not limited to data protection, confidentiality, and all other aspects of information governance.

Ensure the timely management of all communications received and ensuring it is passed to the correct individual best placed to address the patients needs.

Ensure the proper management of patient records, ensuring all correspondence, reports, results etc are filed appropriately and in the correct patient record and summarising and coding is completed as appropriate.

Have a good working knowledge of all practice specific IT systems and general IT systems (Word, Excel etc)

Monitor effectiveness of the system and ensure that action is taken to resolve any problems or effect variations as required.

Liaise with other agencies to ensure timely and appropriate engagement as required.
