Job summary

You will form part of the Practice Clinical Team to deliver effective patient-centric care. You will meet patient health care needs as identified or where care has been transferred to you from a GP. You will need to initiate and maintain health screening and promotion clinics for targeted areas of the Practices patient population, recognising where modification of systems or onward referrals are necessary. You will be able to effectively managing the nursing care of patients with chronic diseases; provide accurate, timely and efficient medical records in line with national NHS Guidelines. You will be responsible for developing and maintaining effective working relationships with all areas of the Primary Care Team and other external agencies to ensure continuity of care.

Main duties of the job

You will have experience in the successful running of clinics covering, but not limited to; immunisations, asthma, diabetic review, hypertension, COPD, vaccinations and cervical cytology.

Routine clinic experience must include;

  • Wound management and suture removal
  • Ordering relevant Blood tests, swabs, MSU when required
  • ECGs and Spirometry
  • Contraception
  • Minor injuries
  • Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary

Job description

Job responsibilities

1. Effectively organise and run clinics in the following areas; childhood immunisations, cervical cytology, asthma, diabetic review, hypertension, COPD, travel and flu vaccinations, wound care and management and suture removal, contraception, minor injuries etc

2. Undertake ECGs and Spirometry as and when required

3. Order and process relevant blood tests, swabs and MSU when required

4. Provide general and specific health screenings to patients within agreed protocols, with referral to other members of the primary and secondary health care team as appropriate

5. Ensure the clinical rooms are kept tidy and in good condition at all times, stocked appropriately whilst maintaining cold storage protocols for effective medicines management

6. Reviewing and monitoring fridge temperature readings and reporting on data logger information

7. Triage patients both virtually and face to face, where appropriate

8. Undertake routine appointments in the following areas; ear care and irrigation, leg ulcer management e.g. Doppler assessment and compression bandaging etc

9. Responding to clinical issues and alerts from outside agencies

10. Managing and co-ordinating changes that need implementing such as; immunisations, introduction of national programmes etc

11. Contribute to and undertake tasks that ensure QOF targets are achieved and conduct patient searches within clinical systems as required

12. Ensure infection control procedures are followed and identify areas for improvement

13. Maintaining vaccination stock levels within the Practice based on demand/need

14. Offer suitable travel health advice by ensuring appropriate vaccines, medications, food hygiene and sun protection information is provided

15. Offer advice about childhood and adult vaccinations and ensure vaccines are administered under Patient Group Directions

16. Undertake relevant administration activities to ensure the smooth running of the Practice

17. Support the team in a clinical crisis and/or emergency

18. Be aware of own impact in the process of care; the need for reflection on processes and resilience when faced with challenges

19. Understand and abide by confidentiality principles at all times

20. Report any incidents that may compromise the health and safety of yourself and others

21. Work in accordance with Equality and Diversity Policy, Data Protection, Health & Safety and organisational dress codes
