Job summary

Munro Medical Centre is a busy, innovative practice committed to professional development. We are seeking a dedicated enthusiastic Minor Illness/Triage nurse who can join us in providing high quality care to our 22,000+ patients.

The ideal candidate will either already be qualified or will have an interest in undertaking advanced clinical practitioner training in the future. Previous Experience desirable in

  • Minor Illness/Triage/A&E
  • Primary Care
  • Chronic Disease Management (with an interest in diabetes)

Main duties of the job

You will assess, diagnose and treat patients with a range of minor illnesses in a GP surgery setting, via face to face and telephone appointments, supported by a large multi-disciplinary practice team, including a daily duty clinician. We have a large group of administration staff to support the medical team.

About us

We work from purpose built premises and have a small branch site in the village of Pinchbeck, we use systmone as our clinical system. We employ 70+ staff and in addition to the nursing team we have a variety of other supporting clinicians and administration staff. We are a busy practice , but we have excellent systems in place tosupport all clinicians in their day to day work.

We have regular practice/team/clinical governance meetings and are committed to patient safety

One thing that is regularly fed-back to us from visitors/GP registrars/Locums etc is that we are a very friendly, supportive team.

Responsible to: Clinical Governance/Team Manager, Practice Manager and Partners at Munro Medical Centre.

Main purpose of Post:

To work alongside medical personnel in the care of patients within the medical centre to include treatments, preventative care, screening and patient education.

To provide a high standard of patient care, making best use of clinical skills within the practice.

To liaise with clinical nurse lead and deputy nurse lead to provide a streamlined service within the medical centre.

To provide support to junior members of the team including Health Care Support Workers and junior nurses.

Good communication and relationships must be established and maintained with patients, colleagues and employers at all times.

Follow guidance and practice policies at all times.

Complete the Minor Illness Diploma and MSc Advanced Clinical Practice

Triage/Minor Illness Duties

To undertake independently face to face and telephone consultations for the assessment and treatments following nursing policy and prescribing formulary.

Refer patients to secondary care/other agencies where appropriate.

Advice related to patient care/treatment choices should be sought by senior members of the nursing team and/or duty clinician.

Issue appropriate treatments following nurse formulary and ensuring prescriptions are signed by GP on-call.

Undertaking face to face and telephone consultations using nursing formulary for drug treatment choices when required.

To undertake data collection for QOF during patient contacts.

To work within nurse prescribing formulary/practice protocol for minor illness as developed by clinical lead and partners.

To be cost effective

To support and facilitate learning to new and existing members of the nursing team.

Practice Nurse Duties:

To undertake the following procedures adhering to practice policy and guidance.

Training will be provided in this area if required and deemed necessary

Patient assessment/minor illness management

ECG recording

D-Dimer testing

Doppler

Blood pressure management including titration of medications within practice policy.

Wound care including traumatic wounds and chronic wound management.

Venepuncture

Injections within scope/practice requirements, not limited to but including for example Zoladex, Flu vaccines, B12, Pneumonia, covid

Contraceptive Advice including depot injection.

Travel Vaccinations and assessment

Child Immunisations and monitoring of immunisation schedules.

Cervical cytology

Spirometry/feno and interpretation.

Chronic Illness reviews

Ordering and stocking up of clinical areas where appropriate.

Undertaking policy reviews and amendments where required.

Swabs labelling and sending to the lab, actioning these results where appropriate.

Assisting when require the clinician in minor surgery in the absence of HCSW.

Other Nursing duties

Inclusion of extended roles and other nursing duties undertaken by senior members of the team.

Insulin Initiation.

Anticoagulation patient management and clinics.

Health Promotion and patient education responsibilities

To manage chronic disease as outlined within the QOF/Primary Care Network framework in accordance to practice policy and NICE guidance.

To help develop nursing policies and update templates for use with systmone computer software.

Diabetes patient education/management and annual reviews

Asthma patient education/management and annual reviews

COPD patient education/management and annual reviews

CHD patient education/management and annual reviews

Health promotion/education

Family planning advice

Pre conceptual advice

Courses will be available to manage these conditions at Diploma/Degree level

Administration and Professional responsibilities

Ensure accurate and legible notes of all consultations and treatments are entered in patients electronic records according to practice policy.

Ensure accurate completion of all necessary documentation associated with patient health care within the practice.

Ensure collection and maintenance of statistical information required for regular ad hoc reports and audit.

Develop practice protocols as required following clinical lead guidance and NICE recommendation including the development in computerised templates.

To undertake incident reports where appropriate.

To undertake safeguarding matters, in accordance with local and national policies

With nursing colleagues undertake risk assessment where applicable.

Attend and participate in both practice and nursing meetings as required.

To undertake research studies independently or in conjunction with other health care professionals with the support and guidance from clinical nurse lead and GPs, with the objective to improve or re-evaluate nursing care and services.

Training and professional development

Training and development will be monitored by yearly appraisal and the formulation of a personal learning plan in conjunction to the practice learning plan .

Professional development will be encouraged and supported by the practice. It is the individuals responsibility to keep their CPD up to date and ensure they are registered with the NMC.

It is the responsibility of the nurse to ensure they develop and maintain a personal learning plan.

Participate with the education and training of students of all disciplines and introduction of all members of the practice staff where appropriate.

If it is necessary to expand the role to include additional responsibilities appropriate training will be given.

To provide cover for colleagues in cases of absence this is including the late night nurse role and cover of some extended hours clinics where appropriate.

To provide additional duties as requested by clinical nurse lead/deputy, practice manager and GP partners.

Communication

As well as the nursing team there is a need to work collaboratively with the multi-disciplinary team, reception and administration staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person/persons.
