Job summary

Abbey Medical Group is looking to recruit a Practice nurse to help support our team on a part-time permanent basis.

Abbey Medical Group is a diverse training practice with 12,000 patients, over two sites based in Ravenshead and Blidworth, Nottinghamshire.

The successful candidates will contribute significantly to the nursing team rota for both the Extended Access services and our core opening hours.

Salary will be discussed at the final interview stage and will be based on previous experience.

Main duties of the job

This forward-thinking GP surgery is offering an experienced Practice Nurse the chance to join their dedicated clinicians, who provide nothing but the best service for all patients. You will be part of the nursing team running a range of different clinics. You will be provided with support, opportunities to develop your career and be surrounded by a team that embody a patient-first ethos.

In addition to the standard Practice Nurse qualifications (Including Travel/Child immunisations and Cervical Screening), you must also be NMC Registered and ideally have a minimum of two years experience within a GP setting.

The post holder will use their own judgement resourcefulness and common sense and show flexibility with regard to rotas and working patterns

Job description

Job responsibilities

The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

Primary responsibilities

a. To develop, implement and embed health promotion and well-being programmes

b. To implement and evaluate individual treatment plans for chronic disease patients

c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

d. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. To provide wound care (ulcer/Doppler etc.) to patients

f. To provide travel medicine services

g. To request pathology services as necessary

h. To process pathology results as required

i. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

j. To maintain accurate clinical records in conjunction with extant legislation

k. To ensure read codes are used effectively

l. To maintain chronic disease registers

m. To develop, implement and embed well woman clinics

n. To develop, implement and embed well man clinics

o. To chaperone patients where necessary

p. To prioritise health issues and intervene appropriately

q. To support the team in dealing with clinical emergencies

r. To recognise, assess and refer patients presenting with mental health needs

s. To implement vaccination programmes for adults and children

t. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

d. Continually review clinical practices, responding to national policies and initiatives where appropriate

e. Participate in the review of significant and near-miss events applying a structured approach, i.e. root cause analysis (RCA)
