Job summary

The social prescriber link worker will provide a holistic approach to healthcare by supporting individuals to improve their overall health and well-being through access to non-medical, community-based support. Social prescribers focus on the wider determinants of health such as loneliness, housing, financial challenges, and lifestyle factors working alongside individuals to address the social, emotional, and practical issues that impact their health.

Main duties of the job

  • Assess patients health and well-being needs and identify support through local services, community resources, and statutory provisions.
  • Co-produce personalised care plans reflecting individual needs, goals, and signposting to relevant services such as weight management.
  • Regularly review care plans to ensure they meet patients evolving needs.
  • Provide personalised support to patients, families, and carers to promote independence and healthy lifestyles.
  • Build trusting, person-centered relationships, focusing on what matters and considering social determinants of health.
  • Support access to services like employment, training, welfare, housing, and personal health budgets as appropriate.
  • Deliver support through face-to-face, virtual, tele health, and tele medicine methods.
  • Promote self-care and assist individuals in accessing funding or services for independence and meaningful engagement.
  • Participate in resolving patient complaints and contribute to learning from incidents.
  • Actively signpost patients to appropriate healthcare professionals or services based on assessed needs

Based within the Brownlow Primary Care Network (PCN) and local GP practices, the Social Prescribing Link Worker connects people to community groups, voluntary organisations, and services to address issues like loneliness, housing, debt, and inactivity.

They will help develop the The Social Prescribing Link service, support PCN priorities, and collaborate with partners to sustain or create local initiatives. This role is especially valuable for people with long-term conditions, mental health needs, social isolation, or complex social challenges.

The Social Prescribing Link Worker will manage their own caseload, promote lifestyle changes, and work closely with a multidisciplinary team. They will receive monthly clinical supervision and have access to support for safeguarding, domestic violence, and mental health concerns. GP referrals will be recorded using SNOMED CT codes in clinical systems.
