Job summary
Brownlow Health is an at-scale Inner City Primary Care
organisation caring for around 80,000 patients across 4 sites. We are looking
to recruit an additional, Band 7/8 (depending on experience) pharmacist to join our pharmacy
team. Currently, this consists of several pharmacists, with two senior leads,
technicians and prescribing clerks. Candidates must be independent prescribers
with relevant experience of Primary Care, and electronic prescribing.
The successful candidate must work effectively as part of large
multidisciplinary team, and provide patient consultations via multiple
modalities.
Promote safe, evidence-based, and cost-effective prescribing in line with local and national guidelines, ensuring adherence to all relevant legislation. Support safe repeat prescribing through protocols, audits, and budget management to reduce waste.

Main duties of the job
Work closely with Brownlow Health's
Prescribing Team, clinical staff, and Frailty team. Engage with the
multidisciplinary team to provide prescribing guidance and make updates to
patient records in collaboration with primary, secondary, and community care.
Conduct structured medication reviews for polypharmacy patients, advise on
prescribing issues, and work to improve patient adherence for better health
outcomes.
Comply
with the CPPE Primary Care Pathway for Pharmacists, if not already completed,
and serve as a Non-Medical Prescriber following GPhC, NICE, and Pan Mersey
guidance. Provide education and support to GPs, practice staff, and patients on
prescribing protocols and best practices.
Lead and participate in audits and clinical governance activities, sharing
findings on prescribing patterns, and contributing to service improvements.
Stay informed on NHS developments impacting medicines management and engage in
local health promotion and equality initiatives.
Maintain accurate records, use the database to manage repeat prescriptions and
patient appointments, and ensure compliance with data security policies. Handle
phone calls and messages efficiently, supporting effective patient
communication and follow-up.
Provide formal and informal training sessions for colleagues and patient
groups. Maintain a portfolio for professional growth.

on-call pharmacy day, supporting the team of GPs with on the day EPS workload
and queries.
The role advertised will further provide support the innovative
Frailty team that Brownlow have established. This will include home visits,
attendance at relevant multidisciplinary team meetings, and providing support
for the wider frailty team around polypharmacy prescribing. Please see attached job description for more information on the role and responsibilities.
Although we allow for some remote working (depending on the role) we want to make you aware that you will need to be based on-site in one of the Practices to carry out your duties. We would also like to state that you will need to reside in Liverpool (or within the near vicinity).
