Job summary

Care Coordinator to support the monitoring of our Long-Term Condition patients and ensure that we adhere to our QOF, IIF and GP Spec requirements.

Working hours will be 32.5 hours over 5 days.

Main duties of the job

Main duties include:

Contact hard to reach LTC patients, check transportation & interpreter needs (via Language Line) and book in with the nurse. Have an active part in the recall process.

Have an understanding of each condition and process. Liaise with leads in each field.

Manage patient appointments on EMIS Web and accurix, ensuring appointments are available and clinics are set up appropriately.

Monitoring all records, notes and letters and extracting and coding information such as BP, foot checks for diabetics, COPD / heart failure clinics info and code accordingly.

Provide information as and when required to service lead to develop service report.

Main Duties and Responsibilities of the Post

In addition to responsibilities listed in the job description please see below:

Follow up patients who did not attend Retinopathy screening.

Code newly housebound patients and arrange DV phlebotomy.

Identify ghost patients and deduct from register.

Liaise with Care homes to obtain relevant info (weight, clinical observations etc) and book pt in for nurse review.

Understand the coding process and the importance of the detail. Communicate regularly with coding team to identify problems when they arise.

Be responsible for weekly updates to Performance & Quality leads and provide input into scheduled team meetings.

Ensure achievement of agreed key performance indicators and provide reports on a regular basis

Provide supervision / direction / training to other team members regarding use of the service as and when required and manage tasks from clinicians and clinical lead

Co-ordinate initial appointment with the clinician / patient ensuring patient has sufficient education prior to appointment, signposting where appropriate.

Ensure appropriate template is used coded correctly supporting clinicians to ensure data is correctly recorded through use of audits and education.

Facilitate any vaccinations as and when required ie B12/Shingles/pneumonia/flu etc for this patient cohort.

Liase with the Quality & Performance Leads in the practice and ensure any outcomes are measured and shared quaterley in a report to the partnership

Any other duty required to support the effective running of the Practices

Work in conjunction with external partners on launching and monitoring new campaigns

Coordinate housebound LTC clinic

Coordinate care home LTC clinics

Coordinate homeless LTC appointments

Communication and relationships

Liaise with members of the Practice, including leads as well as other NHS organisations and outside agencies as appropriate to the role.

Communicate sensitively and effectively with others, using active listening and adapting communication style and method as necessary to meet their needs.

Participate in meetings as appropriate, including regular MDTs with internal and external clinicians and LTC meetings with Quality Lead and Quality Lead Nurse.

Organisation/Quality

Ensure that any data entered into the computer is done accurately.

Support the aims and objectives of the Practice and contribute to the on-going development of the Practice as required.

Practice development and clinical governance

To participate in primary healthcare team meetings when required.

To provide data, as requested, for practice development plans and report.

To be aware of practice policies on risk management / incident reporting and health and safety and operate appropriate procedures for minimising risk.

To be aware of issues related to own development and be committed to participate in training and education.

Training, learning and development

Undergo regular appraisal, supervision and training to update skills and knowledge

Provide training to other staff in work relevant to this post.

Participate in the induction of new staff, students and other attached professionals
