Job summary

The post holder will support both the Homeless and Asylum Team within Brownlow Health both in a clinic environment and also within the homeless hostels. This is a multi- faceted role with a range of clinical duties including monitoring of long- term conditions, new patient health checks, health promotion and health prevention.

Main duties of the job

Clinical

  • Lone visits to homeless hostels
  • Monitoring of long-term condition/annual review - QOF related with complex patients referred to registered nurse
  • New patient health checks
  • Health screening for communicable diseases and give appropriate health promotion advice
  • Administration of Flu, Pneumonia, Shingles, B12 vaccines under PSD
  • Chaperoning duties
  • Vaccine/cold chain storage, monitoring and recording
  • Liaising with and assisting members of the nursing team
  • Accurately perform and record clinical observations including: blood pressure both digitally and manually, temperature, respiration rate, and pulse, SPO2, weight and urinalysis.
  • Have an understanding of their clinical values and know when to escalate.
  • Post Hospital discharge monitoring
  • Low level referrals such as social prescribing, smoking cessation, treatment room
  • Support specialist practitioners- Diabetes, Respiratory.
  • Use of interpreting and translation services.

Management:

  • Assist the clinical team with the process of recalling patients for the management of LTCs
  • Supporting new staff/junior HCAs.
  • Take responsibility for a lead area

Brownlow Health is a large GP training practice committed to supporting our team and developing our services. We have a diverse practice population over six sites in the vibrant City of Liverpool. This post is to work at our Central site at Pembroke Place.

We offer a wide variety of specialist services, including Homelessness, Asylum Seekers/Refugees, Care homes, Student health, Sexual health, Obesity and Frailty and have a large multi -disciplinary team of Nurses, GPs, Pharmacists, Care-co-ordinators, 1st contact physios, Social prescribers, Health coaches, Drugs worker etc.

We have a large supportive nursing team of over 40, managed by three Nurse Managers and a Nurse Partner.

Preference is for this to be a full-time position but part time can be considered.
