Job summary

Due to expansion within our homeless and asylum service we are looking for a committed and dynamic Registered Nurse Associate who has a special interest in working with the homeless and hostel dwelling population in Liverpool as well as a smaller number of asylum seekers. This is a specialist service with dedicated clinicians. As well as working within this team you will also be part of our wider supportive nursing team of 40, managed by two nurse managers and a Nurse Partner with a combination of Advanced Nurse Practitioners (band 7 and 8a), Practice nurses and Clinical Support workers. Main duties of the job

Lone visits to homeless hostels Monitoring of long-term condition/annual review - QOF related with complex patients referred to registered nurse New patient health checks Health screening for communicable diseases and give appropriate health promotion advice Administration of Flu, Pneumonia, Shingles, B12 vaccines under PSD Other Immunisations that require a registered clinician to administer Chaperoning duties Vaccine/cold chain storage, monitoring and recording Liaising with and assisting members of the nursing team Accurately perform and record clinical observations including: blood pressure both digitally and manually, temperature, respiration rate, and pulse, SPO2, weight and urinalysis. Have an understanding of their clinical values and know when to escalate. Post Hospital discharge monitoring Low level referrals such as social prescribing, smoking cessation, treatment room Support specialist practitioners- Diabetes, Respiratory. Use of interpreting and translation services. Cervical Screening - Following Public health guidance on training requirements.
