Job summary

Act as first point of contact for non registered carers and patients who frequently attend practice to establish needs in a non-clinical sense and support in obtaining the correct referrals/ level of support for entitlements and social care. To alleviate pressures on GP capacity, streamlining the patients journey to ensure ongoing care is assessed correctly and planned effectively.

Main duties of the job

Providing advice and support for psychosocial needs and triaging to clinical leads where appropriate.

Proactively identifying new patients for non-clinical assessment and collect data to support decision making in patients pathways for their personalised care.

Although we allow for some remote working (depending on the role) we want to make you aware that you will need to be based on-site in one of the Practices to carry out your duties. We would also like to state that you will need to reside in Liverpool (or within the near vicinity).

Informal Carer criteria

Proactively identify and code new informal carers, performing an initial non clinical assessment and referral to Liverpool Carers Centre and Social Prescribing service.

Liaise with clinical lead any clinical issues or concerns.

Maintain and validate carers registers across sites, re coding anyone who is no longer a carer

Monitor the uptake of flu vaccinations for carers, working with the Care Coordinators for Housebound, Frailty and Dementia to coordinate the administration of the vaccine.

Work closely with the Care Coordinator for BAME and Asylum to ensure carers support is equitable.

Link in with Care Home care co-ordinators and MDT leads to identify informal carers of patients in care homes.

Be the point of contact for Carers support in terms of linking patients with Social Prescribing.

Develop strong links with Liverpool Carers Centre and other support services and agencies, offering advice on available funding, respite & support groups.

Identify ghost patients and deduct from register.

Monitor all records notes and letters and extracting and coding information such as BP, smoking, alcohol, ethnicity and physical exercise information and code accordingly.

Provide information as and when required to Quality and Performance Leads to develop a service report.

Lead on carers support events, linking in with Liverpool Carers Centres to perform carers surgeries

Support dedicated carers telephone line to enable easier access to appointments

Ensure achievement of agreed key performance indicators, providing reports on a regular basis.

Provide supervision / direction / training to other team members regarding use of the service as and when required and manage tasks from clinicians and clinical lead.

Ensure appropriate template is used to code correctly, supporting clinicians to ensure data is accurately recorded through use of audits and education.

We require applicants whose native language is not English to demonstrate their competence in the English language. This can be demonstrated by:

- Providing Certification of English proficiency
