Job summary

Brownlow Health forms part of the Central Liverpool Primary Care Network which aims to positively influence the health and wellbeing of the population of central Liverpool and proactively address health inequalities.

Providing support to the four Brownlow Group Practices in the delivery of Central Liverpool Primary Care Network’s priority areas and key performance indicators.

Job Title: Care Co-Ordinator for Housebound, Frailty and Dementia

Main Purpose of Post: To effectively manage and navigate the specific patient groups and ensure the level of service is maintained . Liase regularly with the performance and quality leads and ensure all targets and KPIs are met .

Responsible to: Perfomance/ Quality Leads

Accountable to: Practice Partners, Clinical leads and Operations Manager

AFC: Band 4

Contract Terms: Full Time / Permanent

Main duties of the job

· Contact Housebound patients (excluding care homes) and arrange home visits for annual reviews

· Code newly housebound patients and arrange Domiciliary Visit phlebotomy when required

· Coordinate the covid and flu vaccinations for housebound patients, following direction from the Quality Lead, in conjunction with the Nurse Managers

· Be responsible for coordinating the care of patients on the Dementia register across sites, including organising their annual reviews according to the protocol.

· Be the point of contact for Dementia support in terms of linking patients with Social Prescribing

· Liaise with Dementia care services and agencies when required. Ensuring they have up to date Dementia registers

· Using the EMIS electronic frailty index (eFi) identify patients 65 and over with moderate and severe frailty, ensuring that they have been assessed annually are correctly coded

Job responsibilities

Main Duties and Responsibilities of the Post

· Contact Housebound patients (excluding care homes) and arrange home visits for annual reviews.

· Code newly housebound patients and arrange Domicilary Visit phlebotomy when required.

· Coordinate the covid and flu vaccinations for housebound patients, following direction from the Quality Lead, in conjunction with the Nurse Managers.

· Be responsible for coordinating the care of patients on the Dementia register across sites, including organising their annual reviews according to the protocol.

· Be the point of contact for Dementia support in terms of linking patients with Social Prescribing.

· Liaise with Dementia care services and agencies when required. Ensuring they have up to date Dementia registers.

· Using the EMIS electronic frailty index (eFi) identify patients 65 and over with moderate and severe frailty, ensuring that they have been assessed annually are correctly coded.

· Using EMIS/IT identify patients with severe frailty who are due an annual medication review and flag up with relevant clinicians.

· Ensure details of patient’s informal carer are recorded.

· Identify ghost patients and deduct from register.

· Monitor ‘All Records’ notes and letters and extracting and coding information such as BP, Smoking, Alcohol, Ethnicity and Physical Exercise information and code accordingly.

· Provide information as and when required to Quality and Performance leads to develop a service report.

· Ensure achievement of agreed key performance indicators.

· Provide supervision / direction / training to other team members regarding use of the service as and when required and manage tasks from clinicians and clinical lead.

· Ensure appropriate template is used to code correctly, supporting clinicians to ensure data is accurately recorded through use of audits and education.

· Facilitate any adhoc vaccinations as and when required ie Covid/B12/Shingles/Pneumonia/Flu etc for these patient cohorts.

· Any other duty required to support the effective running of the Practices.

Communication and relationships

· Liaise with members of the Practice, including leads as well as other NHS organisations and outside agencies as appropriate to the role.

· Work in conjunction with the other Care Coordinators to provide well rounded, intregrated support to our patients.

· Communicate sensitively and effectively with others, using active listening and adapting communication style and method as necessary to meet their needs.

· Participate in meetings as appropriate, including regular MDTs with internal and external clinicians, Quality and Performance Leads.

· Ensure that any data entered into the computer is done accurately.
