Job summary
Brownlow Health forms part of the Central Liverpool Primary Care Network which aims to positively influence the health and wellbeing of the population of central Liverpool and proactively address health inequalities.
Providing support to the four Brownlow Group Practices in the delivery of Central Liverpool Primary Care Network’s priority areas and key performance indicators.
Job Title: Care Co-Ordinator for Housebound, Frailty and Dementia
Main Purpose of Post: To effectively manage and navigate the specific patient groups and ensure the level of service is maintained . Liase regularly with the performance and quality leads and ensure all targets and KPIs are met .
Responsible to: Perfomance/ Quality Leads
Accountable to: Practice Partners, Clinical leads and Operations Manager
AFC: Band 4
Contract Terms: Full Time / Permanent

Main duties of the job
· Contact Housebound patients (excluding care homes) and arrange home visits for annual reviews
· Code newly housebound patients and arrange Domiciliary Visit phlebotomy when required
· Coordinate the covid and flu vaccinations for housebound patients, following direction from the Quality Lead, in conjunction with the Nurse Managers
· Be responsible for coordinating the care of patients on the Dementia register across sites, including organising their annual reviews according to the protocol.
· Be the point of contact for Dementia support in terms of linking patients with Social Prescribing
· Liaise with Dementia care services and agencies when required. Ensuring they have up to date Dementia registers
· Using the EMIS electronic frailty index (eFi) identify patients 65 and over with moderate and severe frailty, ensuring that they have been assessed annually are correctly coded
