Job summary
An exciting opportunity has arisen for a pharmacy
technician to join Central Liverpool PCN's expanding prescribing team and to
work with the member practices which includes the Kensington, Marybone,
Princess Park, Vauxhall, Abercromby. St James, Dingle Park and The Elms. We
wish to expand our pharmacy team to deliver high quality pharmacy services
across our network. The team currently is
comprised of seven pharmacists and two pharmacy technicians.
Main duties of the job
· We are seeking a Pharmacy technician preferably with previous experience of working in primary care.
· This exciting new post comes with the benefit of being situated within some training practices with many years’ experience of training GP’s, Nurses and other health professionals thus providing a support rich environment to establish this new way of working. Rarely in the NHS does an opportunity arise to be part of a brand new team and way of working at the cutting edge of primary care medicine
· The post holder will work as part of the multi-disciplinary team in a patient-facing role.
· To support the Primary Care Network to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medication review, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Job responsibilities
- To support medication review and conduct medicines reconciliation.
- To identify patients suitable for medication changes in line with
the Medicines Management work plan to support QOF, prescribing targets and
CCG projects and subsequently make those changes with the agreement of the
GP or pharmacist, e.g: dose optimisation, formulary switching.
- To assess prescribing data using practice clinical computer systems
and patient records, highlight any recommendations and implement changes
if required, to ensure patients medication records are up to date
- Under the direction of the PCN Pharmacist/GPs, assist with
medication changes from hospital letters and medicines reconciliation for
new patients. To highlight any medication errors/queries and report any
concerns to the PCN Pharmacist/GPs to support local clinical governance
structures eg datix reporting.

To implement initiatives to reduce
waste, improve medicines compliance and ensure best practice is adopted in
accordance with local guidelines, legislation, safety updates e.g NICE, MHRA
- To assist the PCN Pharmacist with any searches, data collection and
letter production required for prescribing changes or drug alerts.
- To facilitate High Risk Drug Monitoring
- To provide information and answer technical medicines management
queries from healthcare professionals and members of the public.
- To help develop and implement medicines management initiatives
through the work of the practice e.g. repeat dispensing, pharmacy MURs.
- In conjunction with the PCN pharmacist deliver training and
education on prescribing and therapeutics in general practice
- To provide pharmacy systems training to new members of the team and
provide leadership to new staff.
- To take an active part in our Covid Vaccination programme when
needed, reconstituting vaccine, managing the cold chain and stock control
- To be able to work as part of a team
- To be able to work independently and have good time management
skills
- Work in a professional manner, complying with confidentiality and
data protection policies
