Job summary

Due to recent expansion an opportunity has arisen in our very popular Brownlow Health. Brownlow Health has a high staff retention rate and an excellent supportive working environment. We are looking to recruit a motivated and enthusiastic Care Coordinator.

Main duties of the job

Manage patient appointments on EMIS Web and accurix, ensuring appointments are available and clinics are set up appropriately.

Manage clinical stock, including reordering, and ensure all equipment is available and clinical room is set up correctly.

Manage tasks from clinicians requesting procedure, send appropriate consent/ information and ensure this is returned in a timely manner.

Support for clinician/patient during/after procedure including training in clinical areas where appropriate (for example height, weight, BP, urinalysis, etc.)

Ensure appropriate template is used coded correctly supporting clinicians to ensure data is correctly recorded through use of audits and education.

Job responsibilities

Liaise with Network Practices to promote services and appointment uptake.

Facilitate training for team members and ensure training log and schedule is up to date.

Provide information as and when required to service lead to develop service report.

Manage waiting lists and management of patient cohort through EMIS or excel spreadsheet.

Liaise with clinical lead as and when required.

Ensure achievement of agreed key performance indicators.

Provide supervision / direction / training to other team members regarding use of the service as and when required.

  • Advanced experience of using word, excel and PowerPoint including ability to use word processing skills, emails and the internet to create simple plans and reports
