Job summary

We are seeking a Band 5and a Band 6 Pharmacy Technician to join our team(salary will be dependent on experience)

We seek a Pharmacy technician preferably with previous experience of working in primary care to join Central Liverpool PCN's team

The team currently comprises of 2 pharmacy technicians, 5 pharmacists (shortly expanding to 7) and 2 senior pharmacists You will be expected to complete the CPPE Primary Care Pharmacy Education Pathway if you havent already done so. Study time and supervision will be provided.

This post is situated within some training practices with many years experience training GPs, Nurses and other health professionals providing a support rich environment to establish this new way of working.

The post holder will work as part of the multi-disciplinary team in a patient-facing role.

Main duties of the job

We are looking for candidates with excellent communication and interpersonal skills, who are also able to demonstrate strong organisational skills, work well independently and as part of a team, and have the ability to prioritise competing demands. You will have the opportunity to become part of GP practice teams and contribute to their prescribing quality and safety. You will also have the opportunity to actively participate in the network pharmacy team, contributing to raising the standards of prescribing across the network.

You should be registered with the General Pharmaceutical Council (GPhC) with BTEC National Diploma in Pharmaceutical Science, NVQ level 3 Pharmacy Services or equivalent.

Experience of a pharmacy technician role in a primary care setting would be desirable although not essential as training can be provided.

  • To liaise with patients, practice staff, GPs, care home staff and community pharmacists in all aspects of medication reviews and prescribing queries to improve the pharmaceutical care of patients.
  • To support medication review and medicines reconciliation.
  • To identify patients suitable for medication changes in line with the Medicines Management work plan to support QoF, prescribing targets and CCG projects and subsequently make those changes with the agreement of the GP or pharmacist, e.g: dose optimisation, formulary switching.
  • To assess prescribing data using practice clinical computer systems and patient records, highlight any recommendations and implement changes if required, to ensure patients medication records are up to date

To implement initiatives to reduce waste, improve medicines compliance and ensure best practice is adopted in accordance with local guidelines, legislation, safety updates e.g NICE, MHRA

  • Under the direction of the PCN Pharmacist/GPs, assist with medication changes from hospital letters and medicines reconciliation for new patients.
  • To highlight any medication errors/queries and report any concerns to the PCN Pharmacist/GPs to support local clinical governance structures eg datix reporting.
  • To assist the PCN Pharmacist with any searches, data collection and letter production required for prescribing changes or drug alerts.
  • To facilitate High Risk Drug Monitoring
  • To provide information and answer technical medicines management queries from healthcare professionals and members of the public.
  • To help develop and implement medicines management initiatives through the work of the practice e.g. repeat dispensing, pharmacy MURs.
  • In conjunction with the PCN pharmacist deliver training and education on prescribing and therapeutics in general practice
  • To provide pharmacy systems training to new members of the team and provide leadership to more junior staff.
  • Work in a professional manner, complying with confidentiality and data protection policies.
