Job summary

An exciting opportunity has arisen for an enthusiastic CLPCN Care Co-ordinator to join our Brownlow family. The Care Co-Ordinator will help to manage frailty, housebound, vulnerable and Asylum Seeker patients

Main duties of the job

To effectively manage and navigate the specific patient groups and ensure the level of service is maintained . Liaise regularly with the performance and quality leads and ensure all targets and KPIs are met.

Job description

Job responsibilities

Manage patient appointments on EMIS Web and accurix, ensuring appointments are available and clinics are set up appropriately.

Provide information as and when required to service lead to develop service report.

Liaise with clinical lead as and when required.

Ensure achievement of agreed key performance indicators and provide reports on a regular basis

Provide supervision / direction / training to other team members regarding use of the service as and when required and manage tasks from clinicians and clinical lead

Support for clinician including training in clinical areas where appropriate (for example height, weight, BP, urinalysis, etc.)

Co-ordinate initial appointment with the clinician / patient ensuring patient has sufficient education prior to appointment, signposting where appropriate

Ensure appropriate template is used coded correctly supporting clinicians to ensure data is correctly recorded through use of audits and education.

Action any non- clinical referrals that arent under the choose and book for any of the identified cohorts of patients E.g: falls service, therapies, assistive technology, telehealth, social px, key safe, LIONS bottles and chase up any referals and link in with services if the patient has failed to attend or declined a service

Formulate a frailty register, check patients have a frailty index score recorded every 12 months and that those coded with severe frailty have had an assessment every 12 months.

Arrange DV phlebotomy for the housebound patients, ring patient and book for a nurse visit when completed

Run searches on overdue housebound bloods and any other outstanding items that could be actioned by nurses (vaccines etc)

Proactively review the practices frail, housebound and vulnerable adult list and cleanse were necessasy and ensure all the patients have a named GP and Nurse.

Escalate to the patients named /regular GP or nurse any issues which need clinical input to improve patients quality of life, establish advanced care planning and prevent admission to hospital

Work with the Social Prescribing Team to signpost patients to suitable external agencies such as Age Concern/Dementia UK etc. Work in conjunction with external organisations such as asylum link to identify patients whose physical and mental health are vulnerable and work with external agencies and the patient to remove barriers to accessing health care across the practices

Facilitate any vaccinations as and when required ie B12/Shingles/pneumonia flu etc for this patient cohort and run searches on patients due these jabs and liaise with meds team to ensure these are prescribed then link in with an HCA to do the home visits or book into HCA clinics were necessary

Liase with the QI lead in the practice and ensure any outcomes are measured and shared quaterley in a report to the partnership

Any other duty required to support the effective running of the Practices

Liaise with members of the Practice, other NHS organisations and outside agencies as appropriate to the role.

Communicate sensitively and effectively with others, using active listening and adapting communication style and method as necessary to meet their needs.

Support the aims and objectives of the Practice and contribute to the on-going development of the Practice as required.

Practice development and clinical governance

To participate in primary healthcare team meetings when required.

To provide data, as requested, for practice development plans and report.

To be aware of practice policies on risk management / incident reporting and health and safety and operate appropriate procedures for minimising risk.

To be aware of issues related to own development and be committed to participate in training and education.

Training, learning and development

Undergo regular appraisal, supervision and training to update skills and knowledge

Provide training to other staff in work relevant to this post.

Participate in the induction of new staff, students and other attached professionals
