Job summary

Band 8A We have a new opportunity and are seeking a highly motivated and experienced Practice Nurse/ Long Term Conditions Nurse with advanced communication skills to lead our team of LTC nurses and drive forward the Quality Outcomes Framework (QOF).

This is a dual clinical and non- patient facing role. As well as working as part of the nursing team managing long term conditions, the post holder will work closely with the Quality Lead, Site managers, nursing team and IT to ensure all our sites are working consistently towards QOF targets and are reached by the desired timescale.

We have a large supportive nursing team of 34, managed by two nurse managers and a Nurse Partner with a combination of Advanced Nurse Practitioners (band 7 and 8a), Practice nurses and Clinical Support workers.

Preference is for this to be a full time position, minimum of 30 hours. Salary and Annual leave is in line with Agenda for Change. We can offer flexible working between 7am and 8pm.

Main duties of the job

This is a new position and exciting opportunity for you to develop this role. The post holder will be a highly experienced Practice Nurse/ Long Term Conditions Nurse.

This is a dual clinical and non- patient facing role. In addition to your clinical role you will also lead and manage our team of LTC nurses and will work closely with the Quality Lead, Site managers, recall team and IT team to ensure all our sites are working consistently towards QOF targets and are reached by the desired timescale. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As an autonomous practitioner the nurse will be a non-medical prescriber able to demonstrate critical thinking and decision-making relating to Long term conditions.

Key responsibilities

Clinical practice

Assess, plan, implement and evaluate individual care plans and treatment programmes for patients with/at risk of a long term condition

Provide a range of clinics including face to face, telephone and eConsultation

Manage and treat long-term conditions including but not limited to: COPD, Asthma, Diabetes, Hypertension, CHD, IGR, in line with national and local policies and practice needs. Prescribe as necessary for these conditions.

Review medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments

Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Implement and participate in vaccination and immunisation programmes for both adults and children including patients travelling abroad

Meet the needs of patients presenting for opportunistic wound care

Communication

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating, including those receiving bad news

Act as an advocate when representing patients and colleagues

Participate in practice team meetings, delivering the nursing agenda and run nurse department meetings

Produce written documents that evidence the contribution of the nursing team to the practice priorities
