Job summary
This role will be part of the Multi-Disciplinary Team (MDT) at Haxby
Group. The post holder will contribute to tackling inequalities in health and
social care. This role will suit an enthusiastic
individual who is able to work collaboratively across a team and keen to
deliver within the evolving role of the network.
The Care Co-ordinator
will be part of a team of care coordinators supporting different cohorts of
vulnerable patients including, but not limited to, residents in care homes,
people with learning disabilities, people with severe mental health illness,
newly diagnosed cancer patients, carers, people from ethnic communities and
people with long term conditions.
The post holder will
work alongside the nursing and medical teams in proactively supporting patients
to engage with annual health checks, developing support plans and signposting
and coordinating support from other agencies to improve inequalities in health.
The
post holder will work across the 4 GP surgeries (Haxby Group Practice
Scarborough, Eastfield Medical Centre, Castle Health Centre, and Brook Square
Surgery) who form Scarborough Core PCN.

Main duties of the job
- Working closely with the
Primary Care Network Transformation Manager.
- To support the social
prescribing team obtaining information from patients and signposting services.
- Working on specific projects
directed by the PCN to address health inequalities.
- To support existing projects to
provide cover as and when required.
- To provide administrative
support for various projects as and when directed.
- To keep accurate records using
agreed systems and contribute to project evaluation.

To work according to
confidentiality and data sharing policies of SCORE PCN.

Achieve agreed performance
targets and participate in regular supervision and personal reviews.
- Engage in training, identify
personal and professional development needs and continuously seek to refresh
knowledge in relation to the job.
- Cooperate in the formulation
and development of systems for monitoring and evaluating the work of the
organisation.
- Provide information for and
contribute to the administration of communications media as appropriate.

Practice's as and when required.

Work within the Clinical
Governance framework of the practices in the PCN, incorporating risk management
and all other quality initiatives.
- Where required and appropriate
refer people back to other health professionals within the primary care
network.

Support staff within the
primary care network with information about local voluntary and community
resources and how patients can access them.
