Job summary

We are looking to expand our Nursing Team with the introduction of a Nursing Associate.

The role is offered as a Full Time / Part Time role - with the working week and hours negotiable.

We have a patient population of around 12,200 patients and cover a demographically diverse population

The role may require some aspects of visiting patients in their own homes for procedures to be carried out.

You would work closely and be supported by our GP Partners, Salaried GPs, Clinical Pharmacist, Practice Nurses, Physician Associate, alongside our Primary Care Network colleagues of Clinical Pharmacists, Social, Health & Wellbeing Practitioners, Occupational Therapist, First Contact Physio and Care Home Teams.

With time and training where needed, the expectation is that the role will develop in line with other priorities, development of the Practice and its offering of services to patients. The ideal candidate for us would therefore be a forward thinker, enthusiastic individual with initiative, keen to work within a team of like-minded colleagues.

We have opportunities for development in areas such as supporting Minor Surgery, long term chronic disease management and Spriometery overtime.

Main duties of the job

We have a patient population of around 12,200 patients and cover a demographically diverse population.

Ideally, we would be looking to recruit a Nursing Associate with experience of being able to carry out for example blood tests, blood pressure monitoring, NHS health checks, Chronic disease monitoring, Learning Disability reviews alongside health promotion, ECGs, administration of routine injections and supporting Minor Surgery clinics. Knowledge of the key indicators that General Practice must work towards would also be an advantage but is not essential.

It is essential that the successful candidate for this role has experience of venepuncture.

The role may require some aspects of visiting patients in their own homes for procedures to be carried out.

In purpose-built premises inside St Annes Health Centre, with a current patient population of around 12,000 patients.

Located in the seaside town of St Annes along the Fylde Coast, we are minutes away from the sea front, with direct links to the M55 & M6.

We are a friendly, professional, caring team who are passionate about providing safe and effective care to our patient population. Our excellent multi-skilled Clinical Team comprises of 8 GPs (Partner & Salaried), , Clinical Pharmacist, Physicians Associate, Practice Nurses & Healthcare Assistants.

Alongside offering the standard contractual requirements of General Medical Services in Primary Care, we do in addition have enhanced bespoke services to our patients in Minor Surgery, Microsuction and are also one of the Covid Vaccination sites within our Primary Care Network.

Parcliffe Medical Centre is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.

Job responsibilities

Job Summary

To provide high quality nursing associate services, which include health promotion to the Practice population.

To deliver and assist clinical staff in the provision of treatment, preventative care and patient education to help support patients improve and maintain their overall health.

To work under indirect supervision and perform tasks prescribed and delegated, working to PSDs and agreed protocols, within an evidence base and within defined competencies to meet the needs of patients and the Practice.

Working to improve standards of care and be involved in identifying practice population needs, supporting the contractual, local and national requirements of specifications as requested e.g GMS Contract, Quality Outcomes Framework and enhanced services.

Key Responsibilities

Clinical Practice

Chaperoning / assisting patients where appropriate who are being examined by another clinician.

To prepare and assist with minor surgical procedures.

To support cold chain management including fridge temperature checks, data logger checks and the raising of cold chain incidents

Making onwards referrals to specialist clinics such as diabetes prevention and education, diabetic eye screening, pulmonary rehab, diet, weight management, smoking cessation, and general lifestyle referrals

Assist in the assessment, planning, development, and implementation of programmes to promote health and wellbeing.

Take observations and identify signs of deterioration and report these appropriately. This may include but is not limited to pulses, respirations, oxygen saturations, peak flow, blood glucose, nutrition, and hydration.

Undertake pathology requests. For example, phlebotomy, urine culture and swabs. Accurately label and document patient samples and submit for collection and analysis.

Undertake investigative procedures including ECG, blood pressure, urine analysis, foot checks, spirometry.

Deliver health promotional health checks such as NHS Health checks.

Contribute to chronic disease management in areas such as but not limited to Asthma, COPD, Diabetes, CVD, Learning Disabilities, CKD and other enhanced service specifications as required appropriate to role.

Provide Cervical cytology, once trained to do so.

Administer routine injections in accordance with the practice policies i.e childhood and adult immunisations in accordance with national guidelines as well as hormone injections, Vit B12 injections, contraceptive injections, flu / covid vaccinations alongside the completion of accurate recording of patient vaccination records. Under PSD as required.

To support the infection, prevention and control lead within the practice to undertake regular audits and oversee required training for staff. Including attendance at meetings and additional training sessions as needed.

Partake in audit as required and identified.

Support training and development of any new or development roles within the team as requested by leads.
